摘要
目的:应用彩色多普勒超声诊断技术,探讨糖尿病肾病(DN)与中医辨证分型的关系;方法:采用彩色多普勒血流显像对100例糖尿病肾病患者及40例正常人的肾段叶间动脉进行检查,并对其血流的收缩期最大血流速度、舒张期最低血流速度和阻力指数进行对比分析;结果:随着DN患者病情的加重,肾动脉收缩期峰值流速、舒张末血流速度及平均血流速度逐渐下降。中医辨证分型按照气阴两虚、阴阳两虚、阳衰瘀阻的规律发展,研究组与对照组各项参数比较均具有统计学差异(P<0.05);结论:肾动脉血流灌注量在DN中医辨证分型中各组间随DN的发展而减少。阻力指数逐渐增加,其值和DN的病情呈正相关。提示随着DN的发展,动脉硬化加重,肾虚证的临床表现和肾缺血的程度有关。
Objective ： To find the relationship of the DN and syndrome differentiation of TCM by the study of color -doppler ultrasonic diagnosis technology. Method： We checked the color - doppler blood - flow imaging of renal artery andleafy artery of 100 cases of patients with DN and 40 cases of normal persons. And then compared and analyzed the data ofthe peak systolic blood flow velocity, minimum diastolic blood flow velocity and resistance index. Result ： With the deterio-ration of the illness, the renal artery peak systolic blood flow velocity, the late diastolic blood flow velocity and the averageblood flow velocity gradually decreased. In the point of syndrome differentiation of TCM ,from the rules like deficiency ofboth qi and yin, yin - yang deficiency and yang failure stasis resistance, we can have statistical differences （ P 〈 0.05 ） bycomparison of the parameters between research group and control group. Conclusion：The volume of the renal artery bloodflow perfusion decreases with the change of DN in the groups on the basis of syndrome differentiation of TCM. The resist-ance index increases gradually while its value is positively correlated with the condition of DN. It results in the fact thatthe change of DN, the aggravation of atherosclerosis and the clinical manifestations of the kidney deficiency relate greatlyto the degree of renal ischemia.
