摘要
目的探讨右美托咪啶对颅内肿瘤手术患者血清S100β蛋白水平的影响。方法全麻下行颅内肿瘤手术患者40例,年龄18-60岁。随机均分为两组,麻醉诱导前10min分别静脉输注右美托咪啶1μg/kg(D组,术中0.5μg·kg-1·h-1持续泵注)和等量的生理盐水(S组)。记录麻醉诱导前(T0)、气管插管后1min(T1)、气管拔管后1min(T2)的MAP和HR。T0、术毕即刻(T3)、术毕12h(T4)和24h(T5)抽取静脉血检测血清S100β蛋白水平。结果与T0比较,S组T1、T2时刻的MAP升高、HR增快(P<0.05),D组T1、T2时的MAP和HR低于S组(P<0.05)。两组T3-T5时的血清S100β蛋白水平均升高(P<0.05),T3-T5时的血清S100β蛋白水平低于S组(P<0.05)。结论右美托咪啶能维持颅内肿瘤患者术中循环稳定,抑制血清S100β蛋白水平的升高。
Objective To investigate the effect of dexmedetomidine on S100β protein of patients undergoing intracranial tumor surgery. Methods Forty patients aged 18-60 years old undergoing selective intracranial tumor surgery under general anesthesia were randomly divided into two groups with 20 cases each. The patients of group D were infused dexmedetomidine 1μg/kg before anesthesia induction, which was followed by pumping dexraedetomidine 0. 5 μg · kg^-1 · h^-1 till 30 min before the end of operation and those of group S were infused normal saline as the controls. MAP and HR were recorded before induction(T0), at 1 rain after intubation(T1) and 1 rain after extubation(T2). Serum concentrations of S100β protein were detected at TO, 6 h(T3), 12 h(T4) and 24 h(TS) after surgery. Results MAP and HR in group S were higher at T1 and T2 than those at T0(P〈0. 05) ,which in group D were lower than those in group S at T1 and T2 (P〈0. 05). Serum concentrations of S100β protein at T3-T5 were higher than those at TO in both groups(P〈0. 05), which in group D were lower than those in group S(P〈0. 05). Conclusion Dexmedetomidine can maintain hemodynamic stability and attenuate the increase of serum S100β protein in the patients undergoing the surgery for intracranial tumors.
