Analysis on the pathogenic bacteria of EICU patients with nosocomial lower respiratory tract infections

摘要 目的探讨急诊科重症监护病房(EICU)患者下呼吸道感染的病原菌构成及药敏试验分析,为指导临床合理应用抗菌药物提供科学依据。方法对2013年10月1日-12月30日EICU发生下呼吸道医院感染的55例患者进行病原菌构成及药敏试验结果分析,采用SPSS11.0进行统计处理。结果 224例EICU患者发生下呼吸道感染55例,发生率为24.55%;下呼吸道感染的患者送检痰标本经细菌学检验共分离出病原菌65株,其中革兰阴性菌171株占78.46%,革兰阳性菌9株占13.84%,真菌5株占7.70%;肺炎克雷伯菌对头孢吡肟、阿米卡星等药物耐药率较低,均为28.57%;铜绿假单胞菌对环丙沙星、左氧氟沙星等药物耐药率较低,均为16.67%。结论临床医师应根据病原菌的构成及药敏分析结果合理应用抗菌药物,以降低医院感染的发生。 OBJECTIVE To investigate the composition of pathogens and analyze the drug sensitivity of the patho‐genic bacteria of EICU patients with nosocomial lower respiratory tract infections ,so as to provide a scientific ba‐sis for rational use of antibacterial agents .METHODS The results of pathogens and drug sensitivity test of 55 pa‐tients with lower respiratory tract hospital infection from Oct .1 ,2013 to Dec .30 ,2013 in our hospital emergency intensive care unit (EICU) were analyzed .SPSS 11 .0 was adopted here to make a statistical treatment .RESULTS Totally 55 in the 224 EICU patients had the infection ,the incidence rate was 24 .55% ;sputum specimens from the 55 cases with lower respiratory tract nosocomial infection were examined and 65 strains of pathogenic bacteria were isolated , including 171 strains of gram‐negative bacteria (78 .46% ) , 9 strains of gram positive bacteria (13 .84% ) ,5 strains of fungi (7 .70% ) .The results showed that the drug resistance rate of K lebsiella pneumoni‐ae to amikacin (28 .57% )was low ;The resistance rate of Pseudomonas aeruginosa to ciprofloxacin and levofloxa‐cin was also low (16 .67% ) .CONCLUSION The clinician should use the antibacterial drugs rationally according to the analysis of composition of t pathogens and drug sensitivity results so as to reduce the incidence of hospital in‐fection .
