慢性硬脑膜下血肿钻孔引流术后复发的相关因素分析 被引量:8

摘要 目的探讨慢性硬脑膜下血肿(CSDH)术后复发的相关因素,为其治疗和预后评价提供依据。方法回顾性分析313例(369侧)成人CSDH患者的临床资料,其中复发19例(19侧)。用单因素χ2检验分析年龄、性别、高血压、糖尿病、凝血功能障碍、CT表现(血肿侧别、密度、形态)及术后颅内积气情况等因素对复发的影响;用t检验比较复发组血肿厚度、术后脑复张率与未复发组的差异。结果高龄(年龄>70岁)、双侧血肿、分隔型血肿和凝血功能障碍是复发的显著危险因素;血肿高密度者的复发率高于低密度和等密度者;血肿厚度及术后脑复张率对复发有影响。结论发病年龄、凝血功能障碍、脑组织的机能状态、血肿的CT表现等因素与术后复发密切相关,术前详细的临床和影像学评估对治疗策略的制订和预后评价具有重要意义。
