摘要
目的比较股骨近端锁定加压钢板(LCP)、动力髋螺钉(DHS)、防旋型股骨近端髓内钉(PFNA)治疗不稳定型股骨粗隆间骨折的疗效。方法选择2011年1月至2013年10月张家港市锦丰人民医院收治的不稳定型股骨粗隆间骨折的患者68例为研究对象,按所采用的不同内固定治疗方法分为LCP组(采用LCP治疗)、DHS组(采用DHS治疗)以及PFNA组(采用PFNA治疗),比较三组患者的手术情况、手术后疗效以及术后并发症的发生情况。结果三组不稳定型股骨粗隆间骨折患者术后关节功能恢复情况比较无统计学意义(H=1.31,P>0.05);PFNA组与LCP组术后并发症发生率分别为4.3%和4.8%,显著低于DHS组的16.7%(P<0.05);LCP组与PFNA组患者在手术时间、术中出血量、术后引流量、术后愈合时间方面比较差异均无统计学意义(P>0.05),但均显著优于DHS组(P<0.05),PFNA组手术切口长度显著短于LCP组和DHS组(P<0.05),而DHS组与LCP组比较无统计学意义(P>0.05)。结论 PFNA、DHS以及LCP治疗不稳定型股骨粗隆间骨折均有效,其中PFNA与LCP在手术时间、术中出血量、愈合时间以及术后并发症方面优于DHS。
Objective To compare the therapeutic effects of locking compression plate（ LCP）,dynamic hip screw（ DHS）,proximal femoral nail antirotation（ PFNA） on treatment of unstable fracture of femurs haft.Methods Total of 68 patients with unstable fracture of femurs haft from Jan. 2011 to Oct. 2013 in Jinfeng People＇s Hospital was retrospectively analyzed,and then divided into three groups according to the treatment methods,LCP group with 21 cases were treated by LCP,DHS group with 23 patients treated by DHS and PFNA group with 24 cases treated by PFNA. The operative situation,therapeutic effects and postoperative complications of the three groups were compared. Results The joint function recovery situation of the three group had no statistically significant difference（ H = 1. 31,P〉0. 05）,the postoperative complications incidence rates of PFNA and LCP group were 4. 3% and 4. 8%,which were significantly lower than that of DHS group（ 16. 7%）（ P〉0. 05）. There were no statistically significant differences between LCP and PFNA on the following aspects： operating time,bleeding,draining and healing time（ P〉0. 05）,but they were both significantly better than those of DHS group（ P〈0. 05）,the surgical incisions of PFNA group were significantly shorter than that of LCP and DHS group（ P〈0. 05）,but there was no statistically significant difference between DHS and LCP（ P〉0. 05）. Conclusion PFNA,DHS and LCP internal fixation treatment of unstable fracture of femurs haft are all effective,but PFNA and LCP are better than DHS in the operating time,intraoperative blood loss,healing time and postoperative complications.
