摘要
目的:观察活动期溃疡性结肠炎患者(ulcera tive colitis,UC)治疗前后外周血及肠黏膜中调节性T细胞(regulatory T cells,Treg)及辅助性T细胞17(T helper 17 cells,Th17)的变化,明确Treg/Th17轴失衡与UC活动度的关系.方法:选取2012-05/2013-12山东省交通医院就诊的UC活动期患者45例及15例健康体检者采用流式细胞术检测15例健康体检者及45例UC患者治疗前和治疗后4 wk外周血中Treg细胞及Th17细胞的频率,免疫组织化学法检测肠黏膜中白介素-17(interleukin-17,IL-17)阳性细胞及Foxp3阳性细胞,各组间比较采用单因素方差分析.结果:UC活动期患者外周血中Treg细胞数明显减少而肠黏膜中明显升高,与正常对照组间差异有统计学意义(P<0.01),治疗后外周血中Treg细胞数量回升;而活动期UC患者外周血及肠黏膜中Th17细胞的数量均明显增加,与正常对照组间差异有统计学意义(P<0.01),病情缓解后T h17细胞明显减少,与正常对照组间相比差异仍有统计学意义(P<0.01).结论:Treg与Th17细胞轴失衡与UC的发病密切相关,有望通过对Treg/Th17细胞轴的调节来达到治疗UC的目的.
AIM: To investigate the proportions of regulatory T(Treg) cells and T helper 17(Th17) cells in peripheral blood and inflamed mucosa of patients with ulcerative colitis(UC) before and after treatment, and clarify the role of imbalance of the Treg/Th17 axis in the development of UC. METHODS: Fifteen healthy control subjects and forty-five active UC patients were enrolled in the current study from May 2012 to December 2013. The proportions of Treg cells and Th17 cells in peripheral blood was determined by flow cytometry before and 4 wk after treatment. Foxp3+ or interleukin-17(IL-17+)cells in inflamed mucosa were detected by immunohistochemistry. Analysis of variancewas used to compare the differences between groups.RESULTS: The proportion of Treg cells significantly increased in the mucosa of patients with active UC, but decreased in peripheral blood compared with controls(P〈0.01). The proportion of Th17 cells significantly increased in peripheral blood and inflamed mucosa of patients with active UC compared with controls(P〈0.01). At remission stage, the proportion of Th17 cells decreased, but was still higher than that in controls(P〈0.01). CONCLUSION: The imbalance of Treg/Th17 cells is involved in the pathogenesis of UC, and it is a potential therapeutic target for UC.
