摘要
本研究探讨卵巢癌组织中CD4+、CD8+、Foxp3+T细胞的分布特征及其与卵巢癌临床病理特征的相关性。采用免疫组织化学法检测41例卵巢癌组织和12例卵巢良性肿瘤组织中的CD4+、CD8+、Foxp3+T细胞数,并对卵巢恶性肿瘤组织癌巢及间质中的CD4+、CD8+、Foxp3+T细胞数与年龄、病理分级、FIGO分期、淋巴结转移、月经史、腹水、单侧/双侧、包膜是否完整、CA125水平及组织学类型之间的关系进行统计学分析。结果显示,1卵巢癌组织中的CD4+、CD8+、Foxp3+T细胞的数量及CD4+T/CD8+T细胞的比值与良性对照组相比有显著性差异(P<0.05)。2卵巢癌组织癌巢内CD4+、CD8+T细胞的数量及CD4+T/CD8+T细胞的比值显著低于间质内(P<0.05)。3卵巢癌癌巢内,CD4+、CD8+、Foxp3+T的数量及CD4+T/CD8+T细胞的比值均与临床分期有关(P<0.05);CD8+、Foxp3+T的数量还与病理分级有关(P<0.05)。4卵巢癌组织间质内,CD8+T细胞的数量与FIGO分期、组织学类型有关(P<0.05);Foxp3+T细胞的数量与病理分级、FIGO分期、淋巴结转移有关(P<0.05);III期、双侧卵巢癌患者CD4+T/CD8+T细胞的比值低于I/II期、单侧卵巢癌患者,差异有统计学意义(P<0.05)。5Foxp3+T细胞和CD8+T细胞在癌巢内及间质内均呈正相关(r=0.516,P<0.05;r=0.430,P<0.05)。综上所述,卵巢癌组织中CD8+、Foxp3+T细胞的数量明显增加,且与FIGO分期呈正相关,提示二者可能共同参与卵巢癌的发生发展;晚期卵巢癌组织中Foxp3+T细胞的数量增多、CD4+T/CD8+T细胞比值下降提示患者卵巢局部的免疫反应受到抑制。
We designed the study to investigate distribution,expression of CD4^+、CD8^+、Foxp3^+T cells in human ovarian cancer tissues and its correlation with clinicopathological characteristics.We obtained tumor samples from 41 ovarian cancer patients and 12 ovarian benign tumor patients.The cell population of CD4^+T,CD8^+T and Foxp3^+T cells in ovarian cancer and ovarian benign tumor tissues were determined using immunohistochemical staining.1Compared to ovarian benign tumor tissues,the amounts of CD4^+T,CD8^+T,Foxp3^+T cells and the ratio of CD4^+T/CD8^+T cells in ovarian cancer tissues have a significantly statistical difference(P〈0.05).2The numbers of CD4^+T cells,CD8^+T cells and the ratio of CD4^+T/CD8^+T cells in the cancer nest were less than those in the stroma with a significantly statistical difference(P〈0.05).3In the cancer nest of ovarian cancer tissues,numbers of CD4^+T,CD8^+T,Foxp3^+T cells and the ratio of CD4^+T/CD8^+T cells was significantly correlated with FIGO stage(P〈0.05);Meanwhile,numbers of CD8^+T,Foxp3^+T cells was significantly correlated with pathological grade(P〈0.05).4In the stroma of ovarian cancer tissues,CD8^+T cell number was significantly correlated with higher FIGO stage,different histological type(P〈0.05);Foxp3^+T cell number was significantly correlated with pathological grade,FIGO stage,and lymph node status(P〈0.05).The ratio of CD4^+T/CD8^+T cells in single tumor patients,stage Ⅰ/Ⅱ patients,was higher than bilateral tumor patients,stage Ⅲ patients,respectively(P〈0.05);5Distribution densities of Foxp3^+T cells and CD8^+T cells correlated positively in both the intraepithelial(r=0.516,P〈0.05)and stromal compartment(r=0.430,P〈0.05)in advanced ovarian cancer patients.Based on these findings,we conclude that numbers of CD8^+T,Foxp3^+T cells in patients with ovarian cancer is significantly elevated,and positively correlated with FIGO stage,this indicates that CD8^+T,
