骨髓间充质干细胞辅助脂肪移植术填充头颈术后腔隙的初步研究

摘要 目的研究骨髓间充质干细胞(MSCs)辅助脂肪移植术后脂肪体积的变化,探索头颈肿瘤术后填充可行性方法。方法分离培养雄性大鼠骨髓间充质干细胞,Wistar雌性大鼠60只,按数字随机法分为对照组和实验组,颗粒脂肪1 ml+0.4 ml DMEM-LG为对照组,脂肪颗粒1 ml+0.4 ml DMEM-LG+1×106MSCs为实验组;均匀震动5 min,移植于背部皮肤与肌肉之间。移植后3、5、7、14、28、60 d每组各取5只脱颈处死,沿被膜分离出移植物。免疫组化法测定CD34观察血管生成,28、60 d测移植物体积,观察其病理改变。结果 28 d时对照组与实验组的脂肪体积保持率无明显差别(P>0.05),60 d时实验组脂肪体积保持率好于对照组(P<0.05),且脂肪结构完好,细胞分化成熟。结论骨髓间充质干细胞辅助脂肪移植术有效保持了移植术后的脂肪体积,为临床应用脂肪移植填充头颈术后腔隙奠定了基础。 Objective To observe the volume change of fat tissue after mesenchymal stem cell( MSC)-assisted lipotransfer,and to explore the feasibility and method of lipotransfer padding for postoperative defect in head and neck surgery. Methods 60 male Wistar rats were randomly divided into control group and MSC group. MSCs of the MSC group were collected and cultured in vitro. Auto-fat grain of the same volume( 1 ml) was obtained from inguinal region in both groups. The fat grain was mixed with both DMEM-LG( 0.4 ml) and MSCs in MSC group,but only with DMEM-LG in the control group. The mixtures were transplanted to back of rats. Fat grafts of 5 rats were obtained from both groups respectively on the 3,5,7,14,28,and 60 days after transplantation. The expression of CD34 in transplanted fat tissue was detected by immunohistochemistry. The volume of fat graft was measured by messcylinder on the day 28 and 60. Results The difference of percentage of survival volume of fat graft between both groups was statistically insignificant( P〉0. 05) on the day 28. But the percentage of the MSC group was significantly higher than that of the control group( P〈0. 05) at the day 60 after transplantation. Conclusion Mesenchymal stem cell-assisted lipotransfer increases the survival volume of fat grafts,which establishes the foundation for lipotransfer padding of postoperative defect in head and neck surgery.
