摘要
目的：探讨经尿道输尿管镜下钬激光碎石术治疗输尿管结石的临床疗效。方法：选取笔者所在医院2013年1月-2014年1月收治的行输尿管镜下钬激光碎石术的102例患者，对其临床资料进行回顾性分析，并总结处理方法、疗效及其并发症。结果：本组102例病例中一次碎石成功率达99.0%。手术时间为20-120 min，平均50 min；输尿管上段结石手术时间较长，为60-120 min，平均87 min。术后住院天数3-5 d，平均4.5 d。术中输尿管穿孔3例，黏膜下假道形成1例。1例因输尿管狭窄行钬激光内切开患者，术后2个月再次出现输尿管狭窄，再次行钬激光内切开术，双J管留置3个月，拔管后随访半年无狭窄发生。结论：经输尿管镜下钬激光碎石术是治疗输尿管结石的一种安全、有效的治疗方法，它具有创伤小、恢复快、并发症少等优点，并可同时进行碎石、切除息肉、处理输尿管狭窄等。
Objective：To investigate the clinical effect of holmium laser lithotripsy transurethral ureteroscope in the treatment of ureteral calculi.Method：A total of 102 patients with holmium laser lithotripsy under ureteroscope in our hospital from January 2013 to January 2014 were selected,and the clinical data were analyzed retrospectively,and the processing method,curative effect and complications were summarized.Result：In 102 patients of this group,the once gravel success rate was 99.0%.The operation time was 20-120 min,average time was 50 min,the upper ureteral calculi operation time was long,it was 60-120 min,the average time was 87 min.Postoperative hospital stay was 3-5 days,the average number of days for 4.5 days.In operation,3 cases were ureteral perforation,1 case was submucosal false passage.One case because of ureteral stricture for holmium laser urethrotomy patients,postoperative 2 months again for ureteral stricture,once again underwent holmium laser urethrotomy.Double J tube indwelled for 3 months,after extubation were followed up for six months without stenosis.Conclusion：Under ureteroscope holmium laser lithotripsy for ureteral calculi is a safe,effective method for the treatment,it has the advantages of small trauma,quick recovery,less complications,can the gravel,removal of polyps,treatment of ureteral stricture.
