摘要
目的:初步评价中药制剂消核糖浆治疗男性乳房发育症的疗效、安全性和经济性。方法:将适合药物治疗的男性乳房发育症患者按照3∶1的比例随机分为单药治疗组和联合用药组;单药治疗组再随机分为3个亚组分别接受消核糖浆、红金消结胶囊或托瑞米芬治疗;联合用药组则随机分为2个亚组分别接受消核糖浆-托瑞米芬、红金消结胶囊-托瑞米芬联合治疗;治疗2个疗程后复查,并进行临床疗效、安全性和经济性的初步评估。结果:消核糖浆治疗男性乳房发育症的有效率为72.7%,与红金消结胶囊或托瑞米芬的疗效相当(P>0.05),但复发率9.43%显著低于托瑞米芬(P=0.029);消核糖浆联合托瑞米芬的有效率为91.3%,与对照组的疗效相当(P>0.05);消核糖浆和红金消结胶囊的不良反应发生率相似(P>0.05),均显著低于托瑞米芬组(P=0.024);成本-效果分析显示,如果获得相同的临床疗效,消核糖浆组的成本低于红金消结胶囊组,其经济性更佳。结论:消核糖浆治疗男性乳房发育症的疗效与两个对照药物相似,但复发率和不良反应发生率比托瑞米芬低,经济性比红金消结胶囊合理,因而在男性乳房发育症的治疗中具有一定的优势。
OBJECTIVE To preliminarily evaluate the efficacy,safety and economy of Xiaohe syrup,a Chinese drugs preparation,in treating of gynecomastia.METHODS The patients with gynecomastia were randomly divided into monotherapy or combination therapy group according to the ratio of 3 vs 1.Then,the monotherapy group was randomly divided into three subgroups and treated with Xiaohe syrup,Hongjinxiaojie capsule and toremifene,respectively,while combination therapy group into two sub-groups and treated with Xiaohe syrup-toremifene or Hongjinxiaojie capsule-toremifene.After two course of treatment,the state of gynecomastia was rechecked and the efficacy,safety and economy of Xiaohe syrup treatment were simultaneously assessed.RESULTS The effective rate of Xiaohe syrup on gynecomastia was 72.7%,which was similar to those of control medicines（Hongjinxiaojie capsule or toremifene,P〉0.05）.But,the recurrence rate of Xiaohe syrup group was 9.43% which obviously lower than that of toremifene（P=0.029）.The effective rate of Xiaohe syrup combined with toremifene was91.3%,which was similar to the control group（P〉0.05）.The incidence of adverse reaction of Xiaohe syrup was 5.19%,which was similar to Hongjinxiaojie capsule（P〉0.05）but significantly lower than toremifene（P=0.024）.Cost-efficacy analysis revealed that the cost of Xiaohe syrup treatment will be lower than Hongjinxiaojie capsule when obtaining similar clinical outcomes.Thus,Xiaohe syrup is an optimal and economical choice for the treatment of gynecomastia.CONCLUSION The effective rate of Xiaohe syrup on gynecomastia is similar to control medicines,but the recurrence rate or incidence of adverse reaction is lower than toremifene.And the economy is better than Hongjinxiaojie capsule.Thus,Xiaohe syrup has certain advantages in treatment of gynecomastia.
