摘要
目的探讨社区中医药综合干预措施对脑卒中后认知障碍患者的疗效。方法根据认知障碍的分组标准将患者分为3层,其中无认知障碍组50例、轻度认知障碍组30例和轻度血管性痴呆组30例。每层患者按照入组顺序随机分为干预组和非干预组,分别比较每层患者干预前后及干预组与非干预组之间的MMSE量表评分、MOCA量表评分和ADL指数评分,评价中医药综合干预对脑卒中后认知障碍的临床疗效。结果无认知障碍、轻度认知障碍、轻度血管性痴呆3组患者中的干预组干预前后,干预组与非干预组之间MMSE评分比较P<0.05;轻度认知障碍组和轻度血管性痴呆组患者干预组与非干预组之间MOCA评分比较P<0.05,差异有统计学意义。结论社区中医药综合干预能提高患者的MMSE和MOCA评分,能在一定程度上改善脑卒中患者的认知功能水平,尤其是对轻度认知障碍和轻度血管性痴呆的患者效果更好。
Objective To investigate the efficacy of community comprehensive TCM interventions on cognitive impairment after stroke. Methods The patients are divided into three layers according to the degree of cognitive impairment,including 50 patients with no cognitive impairment,30 patients with mild cognitive impairment and 30 patients with mild vascular dementia. Patients in each layer are randomly divided into intervention group and non-intervention group according to the sequence of enrollment. We compare MMSE scores,MOCA scores and ADL index scores of patients in each layer before and after the intervention and patients in the intervention group and the non-intervention group. We evaluate the efficacy of community comprehensive TCM interventions on cognitive impairment after stroke.Results The differences of MMSE scores before and after intervention,the intervention group and the non-intervention group of the three layers are all P〈0. 05. The differences of MOCA scores of the intervention group and the nonintervention group of mild cognitive impairment and mild vascular dementia are P〈0. 05,which means the differrences are statistically significant. Conclusion Community comprehensive TCM interventions can improve the scores of MMSE and MOCA and improve the level of cognitive function of stroke patients to some extent,especially for the mild cognitive impairment patients and mild vascular dementia patients.
