摘要
目的观察强心方治疗老年充血性心力衰竭的临床疗效及对血浆脑钠肽(BNP)的影响。方法将120例老年慢性心力衰竭患者随机分为对照组和治疗组,各60例。对照组采用常规西药抗心力衰竭治疗,治疗组在常规西药抗心力衰竭治疗的基础上加用强心方(党参、黄芪、太子参、附子、桂枝、肉桂、鹿角片等)。疗程4周。治疗前后采用超声心动图检测左室射血分数(LVEF)、左室舒张末期内径(LVEDd),生化检测血浆BNP水平,并观察临床疗效及中医证候疗效。结果治疗组在改善临床症状、心功能分级以及提高射血分数、降低脑钠肽水平等方面均优于对照组(P<0.05)。结论加用中药强心方治疗充血性心力衰竭能够更好地缓解心衰临床症状,进一步提高左心室射血分数并降低血浆BNP水平。
Objective To observe the clinical effect of Qiangxin Decoction in treatment of senile congestive heart failure and its influence on plasma brain natriuretic peptide（ BNP）. Methods 120 cases of elderly patients with chronic heart failure were randomly divided into two groups with conventional treatments（ 60 cases in each group）. Besides the conventional treatment,the treatment group members were given Chinese medicine（ Codonopsis pilosula,astragalus,heterophylla,monkshood,cassia twig,cassia bark,antler slice,etc.） treatment additionally compared with the control group. The course of treatment was 4 weeks. These indicators were observed before and after the treatment including left ventricular ejection fraction（ LVEF）,left ventricular end diastolic diameter（ LVEDd）,the plasma BNP level and the score of clinical therapeutic effect and TCM syndrome curative effect. Results The improvement of clinical symptoms,cardiac function classification,ejection fraction and the level of brain natriuretic peptide in treatment group were better than those of control group（ P〈0. 05）. Conclusion Qiangxin Decoction in the treatment of congestive heart failure can better relieve heart failure clinical symptoms,improve left ventricular ejection fraction and reduce the levels of plasma BNP.
