摘要
目的观察经膝后下小切口Othocord线缝合固定治疗后交叉韧带胫骨止点撕脱骨折的手术疗效。方法选取湖南省郴州市第一人民医院自2008年1月-2012年1月收治的18例患者,确诊新鲜后交叉韧带胫骨附着处的撕脱骨折,采用膝后下小切口切开复位Othocord线缝合固定后交叉韧带胫骨附着处骨折块,采用Lysholm膝关节评分表进行主观评价,患者关节稳定性及活动度、膝关节X线检查的结果为客观评价指标。结果 18例患者术后获得随访,随访时间10-16个月,平均13个月,术后均未发现血管、神经损伤,骨折无移位且愈合良好,主观评价优良率为94.4%,客观评价满意率为100%。结论经膝后下小切口切开复位Othocord线缝合后交叉胫骨止点撕脱骨折手术效果满意,术后未见明显并发症,该手术方法操作简便、微创、效果可靠,值得在临床上运用。
Objective To explore therapeutic effects of Othocord suture linefixation treatment with the avulsion fracture in the tibial attachment area of posterior cruciate ligament, through the mini incision behind the knee.Methods 18 cases with the fresh tibial insertion avulsion fractures in the posterior cruciate ligament from January 2008 to January 2012 were diagnosed in Thefirst people＇s Hospital of chengzhou. The posterior cruciate ligament tibial attachment areas of fracture fragments werefixed with Othocord suture line through the mini incision behind the knee. We use the Lysholm knee score table for subjective evaluation and the stability and activity of the joint, knee joint X-ray results for objective evaluation index. Results All the cases obtained follow-up visits, and the visits were made within 10 to 16 months after the treatments, with an average of 13 months. There were no cases with vascular or nerve damage, and all fracture healing was good without displacement and recovered well. The subjective evaluation of the excellent and good rate was 94.4%, the objective evaluation of satisfaction rate of 100%.Conclusion The Othocord suture line fixation treatment, through the mini incision behind the knee, with the avulsion fracture in the tibial attachment area of posterior cruciate ligament has definite clinical effect. No significant postoperative complications were found in all the cases, and the operation is a safe, simple, effective and minimally invasive. So this method is worth being clinically applied and spread.
