摘要 目的应用声脉冲辐射力弹性成像技术(acoustic radiation force impulse,ARFI)对乳腺组织及病灶进行定量及定性分析,评价ARFI在乳腺病灶良恶性鉴别诊断中的价值。方法对101例女性患者共115个病灶术前行ARFI检查,获得乳腺病灶的面积比和剪切波速度,并与术后病理结果相对照,分析面积比值和剪切波速度值对乳腺良恶性的鉴别诊断有无统计学意义。结果术后病理诊断为良性的病灶78个,包括纤维腺瘤31个,乳腺腺病47个;乳腺恶性病灶37个,包括浸润性导管癌33个,原位癌3个,浸润性小叶癌1个。良性病灶的面积比(0.99±0.10)与恶性病灶的面积比(1.45±0.28)相比较,差异有统计学意义(P<0.01);同时良性病灶的剪切波速度值(2.38±0.59)m/s与恶性病灶的剪切波速度值(7.57±2.66)m/s差异也具有统计学意义(P<0.01)。结论 ARFI能对乳腺病灶的硬度进行定量及定性分析,为乳腺良恶性疾病的鉴别诊断提供重要诊断依据。 Objective To apply the acoustic radiation force impulse (ARFI)to qualitatively and quantitatively analyzing breast lesions in order to evaluate the value of ARFI in differentiating the benign/malignant breast diseases.Methods A total of 101 female patients with 115 breast lesions underwent ARFI imaging.The are aratio of the breast lesion and the shear wave speed were obtained and compared with subsequent surgery results to assess the value of the two indexes in differentiation of benign/malignant breast diseases.Results A total of 78 benign lesions and 37 carcinomas were diagnosed on the basis of histological examination after surgeries,including 31 fibroadenomas,47 adenosises of the breast,33 invasive ductal carcinomas,3 carcinomas in situ and 1 invasive lobular carcinoma.The area ratio was(0.99 ± 0.10) in benign lesions and(1.45 ± 0.28) in malignant lesions and there was statistically significant difference(P〈 0.01).Additionally,significant difference was noted in the shear wave speed between the benign and malignant lesions[(2.38±0.59)m/s vs.(7.57±2.66)m/s,P〈0.01].Conclusion ARFI can qualitatively and quantitatively analyze the stiffness of breast lesions and provide the important diagnostic evidence for the differentiation of benign/malignant diseases.
