摘要
目的探讨吸入混合气状态下血氧水平依赖功能磁共振成像(blood oxygen level dependent-functional magnetic resonance imaging,BOLD-fMRI)在前列腺疾病诊断中的应用价值。方法收集2010-09-10-2011-12-31在山东大学山东省医学影像学研究所就诊的正常前列腺志愿者32例、手术和(或)穿刺病理确诊良性前列腺增生(benign prostatic hyperplasia,BPH)患者35例和前列腺癌(prostate cancer,PCa)患者24例,吸入carbogen气体状态下行BOLD-fMRI成像。分别记录前列腺中央腺区(central zone,CZ)、周围带(peripheral zone,PZ)、BPH区及PCa区吸入carbogen气体前后平均信号强度值,以闭孔内肌作为参照,计算吸入carbogen气体后相对信号强度平均增加率并进行分析。结果正常CZ、PZ、BPH区及PCa区信号强度于吸入carbogen气体后均高于吸入carbogen气体前,且对于同一受检者不同组织曲线形态基本一致,但曲线变化幅度存在差异;吸入carbogen气体后正常CZ、PZ、BPH区及PCa区相对信号强度平均增加率分别为(5.07±1.08)%、(2.78±1.13)%、(6.44±2.02)%和(1.77±0.40)%,采用多组独立样本的秩和检验,经两两比较差异均有统计学意义,P<0.05。通过ROC曲线获得PCa相对信号强度平均增加率的最佳诊断域值为2.36%,此诊断阈值敏感度为100.0%,特异度为62.5%;BPH相对信号强度平均增加率的最佳诊断域值为4.35%,此诊断阈值敏感度为97.7%,特异度为39.1%。结论吸入carbogen气体状态下BOLD-fMRI可以反映出正常前列腺CZ、PZ、BPH区及PCa区的血氧水平变化特点,在BPH和PCa的诊断中具有重要价值,以相对信号强度平均增加率对BPH和PCa进行诊断时具有较高的敏感性,但特异性较低。
OBJECTIVE To investigate the value of blood oxygen level dependent-functional magnetic resonance imaging(BOLD-fMRI)induced by carbogen(95%O2/5%CO2)breathing in diagnosis of prostatic diseases.METHODSBOLD-fMRI was performed using a 3.0Tesla superconducting magnetic scanner(GE)in 32 healthy volunteers,35 benign prostatic hyperplasia(BPH)cases and 24 prostate cancer(PCa)cases which proved by histopathology.Different region of interests(ROI)of central zone(CZ),peripheral zone(PZ),BPH and PCa were drawn.The average signal values before/after carbogen breathing were token down and the obturator internus were selected as internal references.The relative increase signal average rates were computed.All data were processed using SPSS 17.0.RESULTS The signal intensities of CZ,PZ,BPH and PCa during carbogen breathing were higher than that of air breathing.The relative increase signal average rates of CZ,PZ,BPH and PCa after carbogen breathing were(5.07±1.08)%,(2.78±1.13)%,(6.44±2.02)%and(1.77±0.40)%respectively.There was statistical significance between every two groups.The best diagnosis threshold of relative increase signal average rate was 2.36%in PCa,and that in BPH was 4.35%.The sensitivity and specificity were 100.0%,62.5% and 97.7%,39.1%in PCa and BPH respectively.CONCLUSIONS BOLDfMRI during carbogen breathing contributes to the diagnosis of BPH and PCa.The relative increase signal average rate has high sensitivity,but low specificity.
