摘要
目的应用超声二维应变成像技术定量评价心肌梗死后左心室局部收缩功能的改变。方法选取2011年9月至2013年1月在齐鲁医院平邑分院心内科确诊为急性心肌梗死并住院14 d以上的患者36例和健康对照者25例作为研究对象,健康对照者行超声心动图检查,36例患者于住院1 d及14 d均行超声心动图检查。并于心尖四腔心切面分别测量室间隔及左心室侧壁的基底段、中间段、心尖段心内膜下心肌纵向收缩期峰值应变及应变率。结果心肌梗死治疗1 d组与健康对照组、心肌梗死治疗14 d组二维常规超声心动图比较,左心室射血分数显著下降(P<0.05),心肌梗死治疗14 d组与健康对照组比较,左心室射血分数有所下降,但差异无统计学意义(P>0.05);三组间两两比较,健康对照组、心肌梗死治疗14 d组及心肌梗死治疗1 d组室间隔及左心室侧壁基底段、中间段及心尖段心内膜下心肌纵向收缩期峰值应变及应变率依次减低,差异均具有统计学意义(P<0.05)。结论超声二维应变成像技术可定量检测心肌梗死后左心室局部收缩功能的改变并定量评估治疗后效果。
Objective To study on the application of two-dimensional strain imaging ultrasound technology in the quantitative evaluation on the change of left ventrlcular regional systolic function of patients with myocardial infarction. Methods A total of 36 cases of patients with acute myocardium infarction admitted in Pingyi Branch of Qilu Hospital from Sep. 2011 to Jan. 2013 and hospitalized for over 14 days were selected as treatment group, another 25 cases of healthy people were selected as control group. All of the cases were taken two-dimen- sional echocardiography detection. All of the 36 patients were taken two-dimensional echocardiography detection on the 1^st and 14^th day after admission. And the interventricular septum,as well as the systolic peak strain and strain rate of base-inferior wall, mid-inferior wall and apex-inferior wall of left ventricle of all the cases on the 1 ^st and 14^th day after admission were tested respectively from the apical four chamber view. Results The left ventricle ejection fraction of the treatment group on the 1 ^st day after admission was significantly lower than that of the control group and the treatment group on the 14^th day after admission( P 〈 0.05 ). And the left ventricle ejection fraction of the control group and the treatment group on the 14^th day after admission both decreased, but the difference was not statistically significant( P 〉 0.05 ). The systolic peak strain and strain rate of base-inferior wall, mid-inferior wall and apex-inferior wall of left ventricle and the interventricular septum of the control group and the treatment group on the 1^st and 14^th day after admission decreased sucesssively, but there were no statisti- cally significant differences( P 〈 0.05 ). Conclusion Two-dimensional strain imaging ultrasound technology can quantitatively evaluate on the change of left ventricular regional systolic function and the curative effect.
