摘要
目的 探讨急性心肌梗死(AMI)患者白细胞计数与心律失常发生率的相关性.方法 选取2009年4月至2012年5月株洲市恺德心血管病医院收治的203例AMI患者作为研究对象,依据白细胞计数结果将患者分为三组:A组(白细胞计数<8.0 ×10^9L^-1)45例,B组(白细胞计数8.0×10^9L^-1~16.0×10^9L^-1)109例,C组(白细胞计数>16.0×10^9L^-1)49例.分析上述三组患者血清心肌肌钙蛋白I(cTnI)、肌酸激酶MB同工酶(CK-MB)的检测结果,以及各组患者心律失常的发生率.结果 C组患者血清cTnI、CK-MB水平显著高于B组(P<0.05),B组患者血清cTnI、CK-MB水平显著高于A组(P<0.05).C组患者窦性心动过速、心房颤动、室性期前收缩、室性心动过速、心室颤动、房室传导阻滞、束支传导阻滞的发生率显著高于B组(P<0.05),B组患者上述类型心律失常的发生率显著高于A组(P<0.05),且AMI患者白细胞计数与上述类型心律失常的发生率呈正相关(P<0.05).结论 AMI患者白细胞计数与多数类型心律失常的发生率呈正相关,临床应重视AMI患者白细胞升高的检测.
Objective To explore the correlation between the white blood count (WBC) and incidence rates of arrhythmia in patients with acute myocardial infarction (AMI). Methods A total of 203 cases of patients with AMI admitted in Kaide Cardiovascular Disease Hospital from Apr. 2009 to May 2012 were selected and divided into 3 groups according to WBC :45 cases as group A ( WBC 〈 8.0 ×10^9L^-1), 109 cases as group B((8.0-16.0) ×10^9L^-1 〈 WBC 〈 16.0 ×10^9L^-1 ) and 49 cases as group C( WBC 〉 16.0×10^9L^-1) ,respectively. The levels of serum cardiac troponin 1 (cTnl) and creatine kinase MB isoenzyme ( CK-MB), as well as the incidence rates of arrhythmia of the 3 groups were analyzed retrospectively. Results The levels of serum cTnl and CK-MB of group C were significantly higher than those of group B (P 〈 0.05 ), and such levels of group B were significantly higher than those of group A ( P 〈 0.05 ). The incidence rates of sinus tachycardia, atrial fibrillation, premature ventricular beats, ventricular tachycardia, ventricular fibrillation, atrioventrieular block,bundle branch block of group C were significantly higher than those of group B ( P 〈 0.05 ), and such rates of group B were significantly higher than those of group A ( P 〈 0.05 ). WBC of AMI patients was positively correlated with the incidence rates of those types of arrhythmia mentioned above (P 〈 0.05). Conclusion There is a positive correlation between WBC and most types of arrhythmia in patients with AMI, so AMI patients with leukocytnsis should be paid more attention to in clinical practice.
