摘要
目的分析不同联合手术方式治疗青光眼合并白内障的疗效,探讨各种手术治疗的临床价值。方法随机选取2011年4月到2012年12月西昌市人民医院五官科收治的青光眼合并白内障患者100例,按照治疗方式不同分为观察组和对照组各50例,两组患者均行人工晶状体置入术和青光眼小梁切除术,其中观察组另行白内障超声乳化摘除术,对照组另行白内障囊外摘除术。对比两组患者手术治疗的临床疗效和术后并发症情况。结果与术前相比,术后观察组和对照组视力均显著升高[0.12±0.05 vs0.67±0.21;0.13±0.04 vs 0.63±0.19],观察组和对照组眼压均显著降低[(25.67±7.54)mm Hg vs(12.17±5.71)mm Hg;(25.65±7.31)mm Hg vs(13.27±5.03)mm Hg],差异均有统计学意义(P<0.05);术后观察组患者角膜散光显著低于对照组(1.31±0.36)D vs(6.42±2.03)D(P<0.05)。观察组术后并发症发生率显著低于对照组(16%vs 38%)(P<0.05)。结论青光眼白内障联合手术治疗在临床上具有十分重要的意义,白内障超声乳化摘除术在临床上具有较好的疗效,值得临床推广使用。
Objective To analyze the clinical efficacy of different surgical methods in glaucoma with cataract, to explore clinical value of a variety of surgical treatments. Methods Randomly selected from Department of Ophthalmology and Otorhinolaryngology of Xichang People＇s Hospital during Apr. 2011 and Dec. 2012,100 patients with glaucoma and cataract were included in the study, according to the treatment they were divided into observation group （50 cases ） and control group （50 cases ） , both groups received intraocular lens surgery and glaucoma trabeculectomy, while the observation group also received cataract ultrasonic emulsification extraction, and the control group received cataract extracapsular extraction. The clinical curative effect of surgical treatment and postoperative complications of the two groups were compared. Results After the surgery, sight of observation group and control group were significantly increased than before operation[0.12 ± 0.07 vs 0.67 ± 0.21 ;0.13 ± 0.07 vs 0.63±0.19 ], intraocular pressure were significantly decreased than before operation[ （25.67±7.54） mm Hg vs （ 12.17±5.71 ）mm Hg; （25.65 ± 7.31） mm Hg vs （13.27 ± 6.03） mm Hg] （P 〈0.05）. After the surgery,corneal astigmatism of the observation group was significantly lower than that of the control group （1.31 ± 0.56） D vs （6.42 ± 3.03） D （ P 〈 0.05 ）. After the surgery,the incidence of complications of the observation group was significantly lower than the control group（ 16% vs 38% ,P 〈 0.05 ）. Conclusion Cataract combined with glaucoma surgery, has a great significance in clinical, and ultrasonic emulsification and cataract extraction has better curative effect, thus is worth of clinical promotion.
