摘要
目的通过对我院10年内儿童跖骨骨折进行回顾性调查,分析其流行病学特征。方法利用医学影像计算机存档与传输系统(PAcs)收集2003年1月至2012年12月10年间我院放射科诊治的所有骨折患者资料,剔除复查骨折、病理骨折、陈旧骨折,筛选出儿童跖骨骨折,统计性别、年龄、骨折部位等,总结其流行病学特征并进行分析。结果筛选出儿童跖骨骨折370例,占儿童足部骨折的40.66%,男性多于女性(男女比例1.57:1),其中单发跖骨骨折256例,多发跖骨骨折114例共548例骨折部位。11~15岁年龄段构成比最高(45.95%);单发跖骨骨折中第五跖骨骨折最多,占53.13%;而在多发跖骨骨折中累及第三跖骨最多,占28.77%。多发骨折男性比例明显高于单发骨折(P〈0.001)。5岁以下第一跖骨骨折最多,占26.98%;5岁以上第五跖骨骨折最多,占35.07%。跖骨基底骨折最多,占50.73%。98.25%的多发跖骨骨折发生在相邻跖骨之间,77.78%的第三跖骨骨折伴随第二或第四跖骨骨折。结论对儿童跖骨骨折回顾性调查分析有助于儿童跖骨骨折的诊断治疗。
Objective To investigate the pediatric metatarsal fractures within the last decade and analyze their epidemiological characteristics. Methods The clinical data of fractures treated between January 2003 and December 2012 at our hospital were collected through the PACS(Picture Achieve Communication System)system after excluding re-examined, pathologic and old fractures. The pediatric metatarsal fractures were selected with regards to gender, age and fracture site. The epidemiological characteristics of pediatric metatarsal fractures were collected and analyzed. Results A total of 370 pediatric metatarsal fractures cases accounted for 40. 66~/oo of all pediatric foot fractures. There were single (n = 256) and multiple (n = 114) metatarsal fractures. The ratio of male and female was 1.57 : 1. A total of 548 metatarsal fracture sites were classified. Most metatarsal fracture patients, in the 11-15-year-old age group, accounted for 45.95% of all pediatric metatarsal fracture cases. The fifth metatarsal fractures, as the most common in single metatarsal fractures, accounted for 53. 13% of single metatarsal fractures. The third metatarsal fractures, as the most common in multiple metatarsal fractures, accounted for 28. 77% of multiple metatarsal fractures. The proportion of male in multiple metatarsal fracture was higher significantly than that in single metatarsal fracture (P〈0. 001). The first metatarsal fractures accounting for 26. 98% of all metatarsal fractures were the most common in children aged under 5 years whereas the fifth metatarsal fractures accounting for 35.07% of all metatarsal fractures were the most common in children who were over five years old. The metatarsal base fractures were the most common, accounting for 50. 73% of all metatarsal fractures 98. 25% of multiple metatarsal fractures occurred in adjacent metatarsals.
