摘要
目的探索睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)对原发性高血压(高血压)患者心脏结构的影响。方法回顾性分析高血压患者247例的临床资料,根据多导睡眠监测结果将其分为OSAHS-高血压组75例和单纯高血压组172例,比较两组患者的基线资料。分析OSAHS-高血压患者的心脏结构与睡眠呼吸紊乱指数(apnea hypoventilation index,AHI)之间的相关性,并采用多元线性回归模型对影响心脏结构的主要影响因素进行筛查。结果基线资料分析显示,OSAHS-高血压组男性构成比、体质量指数、三酰甘油、室间隔厚度(interventricular septal thickness,IVST)、左心室后壁厚度(left ventricle posterior wall thickness,LVPWT)明显大于单纯高血压组,差异有统计学意义(P<0.05);而血清高密度脂蛋白胆固醇浓度明显低于单纯高血压组,差异有统计学意义(P<0.05)。相关性分析显示AHI同IVST密切相关(r=0.242,P=0.041),而与LVPWT无明显相关性。多元线性回归分析表明AHI与体质量指数是IVST增厚的危险因素。结论呼吸睡眠暂停可能参与高血压患者心脏结构的改变。
Objectives To explore the effects of obstructive sleep apnea hypopnea syndrome (OSAHS) on changes of cardiac structure in patients with hypertension.Methods Clinical data of 247 patients with hypertension were analyzed.The patients were divided into OSAHS-hypertension group and simple hypertension group according to the results of polysomnographic examination.Baseline data of the two groups were compared.Correlations of apnea hypoventilation index (AHI) and heart structure were analyzed.Linear regression model was applied to select the main influence factors of heart structure.Results Baseline data analysis showed that constituent ratio of male,body mass index (BMI),triacylglycerol,interventricular septal thickness (IVST) and left ventricle posterior wall thickness (LVPWT) in OSAHS-hypertension group were significantly higher than those in simple hypertension group (P〈0.05),serum concentration of high-density lipoprotein cholesterol was significantly lower than that in simple hypertension group (P〈0.05).Correlation analysis showed that AHI was closely associated with IVST (r=0.242,P=0.041),but not with LVPWT.Linear regression showed that AHI and BMI were risk factors of thickening of IVST.Conclusions OSAHS may be an influence factor of changes in heart structure in OSAHS-hypertensive patients.
