摘要
目的观察和评价吻合器痔上黏膜环切钉合术(Procedure for Prolapse Hemorrhoids,PPH)联合外痔切剥治疗重度混合痔的临床疗效。方法回顾性分析应用传统PPH术(对照组)及联合外痔切剥术(治疗组)分别治疗重度混合痔40例患者的临床疗效。观察两组术后并发症:术后出血、肛门坠胀、肛门疼痛、排尿障碍;对比两组患者手术时间及住院时间。结果两组手术方式在手术时间、术中出血量、术后当日疼痛及住院天数的比较中P>0.05,均未见明显统计学差异;术后10天观察两组患者的临床比较发现,在肛门瘙痒、便血、肛门不适感上两组比较P>0.05,均未见明显统计学差异。但在肛门疼痛、肛门潮湿、肛缘肿物突出、肛门坠胀感及里急后重感比较中P<0.05,存在统计学差异;术后2月观察两组患者的临床比较发现,在肛门瘙痒、便血、肛门不适感、肛门疼痛、肛门潮湿、肛门坠胀感及里急后重感上两组比较P>0.05,均未见明显统计学差异。仅在肛缘肿物突出比较中P<0.05,存在统计学差异。结论联合使用PPH术及外痔切剥术为传统PPH手术治疗重度混合痔术后存在的肛缘残留皮赘提供一种有效的解决方法。
Objective To observe and evaluate the Procedure for Prolapse , Hemorrhoids joint external piles cut strip clinical curative effect for the treatment of severe mixed hemorrhoid.Mtehods A retrospective analysis the clinical cura -tive effect of the control group and treatment group.Observed two groups of postoperative complications , postoperative bleeding , anal belly , anal pain , micturition obstacles;Compared to two groups of patients with operation time and hospi-tal stay.Results Two groups of surgical methods in operation time , intraoperative blood loss , postoperative day pain and the comparision of hospitalization days （ P〉0.05 ） , no significant statistical difference;After 10 days to observe clinical comparison of two groups of patients , found that in the anus itching , hematochezia , anal discomfort in two groups （ P〉0.05）, no significant statistical difference.But in the damp, anal anal pain, anal edge neoplasm is prominent, the anus sank bilge feeling and feeling of tenesmus comparison （ P〈0.05） , statistical differences;February observed two groups of patients with postoperative clinical comparison found that in the anus itching , hematochezia , anal discomfort , anal pain, anal wet, anus belly on the sense and feeling of tenesmus comparing the two groups （P〉0.05）, no significant statistical difference.Only in the anal edge neoplasm outstanding comparing （ P 〈0.05 ） , statistical differ-ences.Conclusions Combination of PPH surgery and external piles cutting stripping technique for the traditional PPH surgery to treat severe mixed hemorrhoid postoperative anal edge of the existence of residual skin fat provides an effective solution.
