摘要
目的分析心力衰竭合并心房颤动的关系,探讨缬沙坦联合苯那普利治疗心力衰竭合并心房颤动的临床疗效。方法选取隆回县人民医院自2012年5月～2013年5月收治的74例心力衰竭合并心房颤动患者作为研究对象,随机将其分为2组（n=37）,2组患者均针对原发病采取相同的治疗措施,在此基础上,对照组应用苯那普利,实验组在应用苯那普利的同时,加用缬沙坦。结果治疗后实验组患者NYHA分级为（2.0±0.7）,对照组为（2.3±0.7）,LVEF分别为（47.1±14.1）和（42.0±13.3）,HR分别为（80.6±10.9）和（97.8±10.5）,SBP分别为（13.1±1.6）和（15.3±1.8）,DBP分别为（8.9±0.8）和（9.5±0.7）实验组的NYHA分级、HR（heart rhythm）、SBP（Systolicbloodpressure）、DBP（Diastolicbloodpressure）均显著低于对照组（P〈0.05）,LVEF显著高于对照组（P〈0.05）;实验组有4例复发心房颤动,复发率为10.8%,对照组有9例复发房颤,复发率为24.3%,组间比较,差异有统计学意义（P〈0.05）。结论心力衰竭与心房颤动有着密切的关系,对于心衰合并房颤的患者,应用缬沙坦联合苯那普利进行治疗干预,可有效改善患者的心功能,减少房颤的发生,提高病人的生活质量。
Objective To analysis the relationship between heart failure with atrial fibrillation, investigate valsartan combined benzene that split the clinical curative effect of treatment of heart failure with atrial fibrillation.Methods 74 cases with atrial fibrillation in patients with heart failure as the research object, the random divided into two groups, two groups of patients were treated for primary disease to take the same measures, on this basis, the control group used benzene that split, the experimental group in the application of benzene that split at the same time, combined with valsartan.Result After treatment, the experimental NYHA classification, HR, SBP, DBP were significantly lower than control group （P〈0.05）, and LVEF was significantly higher than that of control group（P〈0.05）; The experimental group had 4 cases of recurrence of atrial fibrillation, therecurrence rate was 10.8%, control group there are 9 eases of recurrence of atrial fibrillation, recurrence rate was 24.3%, the comparison between groups, the difference was statistically significant（P〈0.05）.Conelusion Has close relationship with atrial fibrillation and heart failure in patients with heart failure combined af, application of combined benzene that valsartan and captopril treatment intervention, which can effectively improve the patient＇s heart function, reduce the occurrence of atrial fibrillation, improving patient＇s quality of life.
