摘要
目的通过对PHC患者的血清HCY和AFP-L3、AFU水平的联合测定,评价3者联合测定在PHC诊断中的重要意义。方法选取本院2012年-2013年2年来收治的PHC患者194例,作为PHC组;良性肝病患者165例,作为良性肝病组;另选健康体检者92例,作为正常对照组。采用贝克曼DXI800全自动化学发光分析仪,用微量离心柱法分离得到AFP-L3,用化学发光法测得AFP-L3、血清AFP总量,计算出AFP-L3比值即为AFP-L3占血清AFP总量的百分比;采用西门子ADVIA2400型全自动生化分析仪测定AFU和HCY。结果良性肝病组、PHC组与正常对照组比较,PHC组与良性肝病组比较,血清HCY和AFP-L3、AFU水平明显升高;联合检测HCY和AFP-L3、AFU的敏感性、特异性、准确性、阳性率均明显高于单独检测几者;相关性分析结果表明,血清HCY和AFP-L3、AFU水平呈正相关(分别为:r=0.57,P<0.05;r=0.64,P<0.05)。结论血清HCY和AFP-L3、AFU水平是诊断PHC的重要指标,几者联合测定在PHC诊断中具有重要意义。
Objective Through the joint determination of the level of primary hepatic carcinoma( PHC) patients' homocysteine( HCY),AlPha-fetoprotein heterogeneity-L3( AFP-L3),and a-L-fucose-glucoside( AFU) to evaluate the significance of the joint determination of three levels in the PHC diagnosis. Methods In the recent 2 years( 2012- 2013) 194 patients with PHC were selected from our hospital as the PHC group; 165 patients with benign liver disease as the benign liver diseas group;and 92 healthy persons as normal group for comparison. With the Backman DXI800 automatic chemiluminescence analyzer AFP- L3 was isolated by microspincolumn method,the total quantity of AFP- L3 and serum AFP are measured by chemiluminescence,the calculated AFP- L3 ratio was the percentage of AFP- L3 to the total serum AFP; to determine AFU and HCY with Siemens ADVIA2400 automatic biochemical analyzer. Results Through the comparison of benign liver disease group,PHC group and the normal group,as well as the comparison of PHC group and benign liver disease group,the levels of serum HCY,AFP- L3 and AFU increased obviously; in the jointed detection,the sensitivity,specificity,accuracy and positive rate of HCY and AFP- L3,AFU were obviously higher than any separate detection. The correlation analysis results showed that the level of serum HCY,AFP- L3 and AFU were positive related( respectively: r = 0. 57,P〈0. 05; r = 0. 64,P〈0. 05). Conclusion The levels of serum HCY,AFP- L3 and AFU are important indexes for the diagnosis of PHC,the jointed determination has important significance in the diagnosis of PHC.
