摘要
目的评价芪明颗粒治疗糖尿病肾病(DN)的有效性,并探讨其作用机制。方法选取早期DN病人105例,随机分为对照组(52例)和治疗组(53例)。两组均应用降糖药物治疗,并坚持糖尿病饮食、运动锻炼,空腹血糖(FBG)〈7.0mmol/L、餐后2h血糖(PBG)〈8.5mmol/L后,治疗组在此治疗基础上加用芪明颗粒每次4.5g每天3次开水冲服,治疗3个月。治疗前后分别检测两组病人FBG、糖化血红蛋白(HbA1c)、尿蛋白/尿肌酐(ACR)、胱抑素C(CysC)、尿微量清蛋白排泄率(UAER)、β2微球蛋白(β2-MG)、血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)等指标变化。结果治疗前两组FBG、HbA1c、TG、TC、LDL-C、ACR、CysC、UAER、β2-MG水平差异无显著性(P〉0.05)。治疗3个月后,对照组FBG、HbA1c、TG、TC、LDL-C、ACR、CysC、UAER、β2-MG水平与治疗前比较差异无显著性(P〉0.05);治疗组以上指标与治疗前比较明显改善(t=12.706-63.657,P〈0.05),与对照组比较明显降低,差异有统计学意义(t=6.965-31.599,P〈0.05)。结论芪明颗粒能显著改善早期肾脏损伤以及血脂、血糖等指标,在早期DN治疗过程中具有较高的有效性。
Objective To evaluate the efficacy of QI MING granule in the treatment of diabetic nephropathy(DN)and explore its mechanism. Methods The present study consisted of 105 patients with early DN,who were randomized to control group(n=52)and treatment group(n=53).All the patients were treated with hypoglycemic drugs,diabetic diet and physical exercise.After the fasting blood glucose(FBG)reached〈7.0mmol/L,PBG〈8.5mmol/L,QI MING granule 4.5g was then offered three times a day,taken orally with water,for three months.Before and after treatment,the following items were detected in patients of the two groups:FBG,HbA1 c,the urine protein/creatinine(UP/UC),cystatin C(CysC),urinary albumin excretion rate(UAER),beta 2-microglobulin(β2-MG),total cholesterol(TC),low density lipoprotein(LDL)and triglyceride(TG).Results Before treatment,the differences of levels of FBG,HbA1 c,TG,TC,LDL,UP/UC,CysC,UAER andβ2-MG between the two groups were not significant(P〉0.05).After three months of treatment,the differences of the above items were not significant in patients of the control group as compared with that before treatment(P〉0.05),and that in the treatment group obviously improved(t=12.706-63.657,P〈0.05),and the levels of the above items much declined versus the control,the differences being statistically significant(t=6.965-31.599,P〈0.05). Conclusion QI MING granule can markedly improve renal function,blood fat and blood sugar and other markers in patients with early diabetic nephropathy,which is of a higher efficacy in the therapy of this disese in the early stages.
