摘要
目的:分析尿道损伤引起性功能障碍的原因并评价尿道损伤及修复重建手术对性功能的影响。方法:回顾性分析我院泌尿科于2010年1月至2013年1月收治的150名尿道损伤与修复重建手术患者的临床资料,对比前尿道损伤患者与后尿道损伤患者的术前和术后勃起功能障碍的发病率。结果:1前尿道损伤组患者术前勃起功能障碍发生率为46.67%,术后1周勃起功能障碍发生率为49.33%,术后1年勃起功能障碍发生率为29.33%;2后尿道损伤组患者术前勃起功能障碍发生率为54.67%,术后1周勃起功能障碍发生率为52.00%,术后1年勃起功能障碍发生率为38.67%;3两组术前与术后1周的勃起功能障碍发生率无统计学差异(P>0.05),术后1年的勃起功能障碍发生率要明显小于治疗前,差异具有统计学意义(P<0.05)。结论:造成尿道损伤及修复重建手术中勃起功能障碍的原因有血管性、神经性、心理性三方面,尿道端端吻合术并不会加重或增加勃起功能障碍的发病率,术后远期有部分患者可恢复勃起功能。
Objectives：To analyze the sexual dysfunction causes of urethral injury and evaluate the impact of urethral injury and reconstruction surgery on sexual function.Methods：Retrospective analysis on the clinical data of 1 50 patients received urethral reconstructive surgery in our hospital from January 201 0 to January 201 3 was con-ducted.Morbidity of erectile dysfunction before and after surgery of patients with anterior urethra injury and posterior urethra injury was compared.Results：①in anterior urethra injury group,the incidence of erectile dysfunction before operation,1 week and a year after operation was 46.67%,49.33%and 29.33%respectively.②in the poste-rior urethra injury group,the incidence of erectile dysfunction before operation,1 week and a year after operation was 54.67%,52.00%and 38.67%respectively;③ the incidence of erectile dysfunction of the two groups before and one week after the surgery was of no significant difference （P〉0.05 ）.The incidence of erectile dysfunction 1 year after the operation was significantly less than it before treatment.The difference was statistically significant （P〈0.05）.Conclusion：The causes of erectile dysfunction during urethral injury and reconstruction surgery are vascu-lar,neurological and psychological reasons.Urethral anastomosis does not aggravate or increase the incidence of e-rectile dysfunction.Some patients could restore erectile function a long time after operation.
