Analysis on current situation of hospital acquired infections in 2013 in tertiary general hospital

摘要 目的通过对三甲综合医院2013年1-12月医院感染的现状进行调查分析,结合医院实际情况针对调查结果提出干预措施,以有效控制和减少医院感染的发生。方法 2013年1-12月由医院感染控制科专职人员对全部出院病历进行回顾性调查,与临床医师主动诊断报告相结合,填写统一的《医院感染病例调查登记表》,整理相关数据,并对各科室患者发生感染例数、感染部位和感染病原菌分布进行统计、描述和分析,数据采用SPSS13.0软件进行处理。结果 2013年1-12月共调查住院患者27 315例,发生医院感染529例,感染率为1.94%;内科系统感染前3位科室分别为血液移植科、重症医学科和呼吸内科,感染率依次为13.68%、8.48%、3.90%;外科系统感染前3位科室分别为烧伤科、心胸外科、血管外科,感染率依次为5.30%、3.04%、2.78%;呼吸系统、泌尿系统和血液系统为主要感染部位,分别占35.16%、23.25%、19.85%;检出病原菌革兰阳性菌占45.77%、革兰阴性菌占31.51%、真菌占22.72%。结论应有效落实感染控制措施,加强对重点科室、重点人群的监管,减少侵入性操作,合理使用抗菌药物,多角度、多要素、多环节控制,以减少医院感染的发生。 OBJECTIVE To investigate and analyze the current situation of hospital acquired infection in Jan. -Dec .2013 in a tertiary general hospital, and provide interventional measures based on real situations so as to control and decrease the infection rate effectively .METHODS All cases of inpatients discharged from Jan .to Dec. 2013 were reviewed by the specialists in the infection control department .The Record of Case Investigation on Hospital Acquired Infection was completed based on the review and the initiative diagnosis report by clinicians. Relevant data were sorted to statistically describe the number of infected patients as well as the infection sites and strains of infectious pathogens .Data were processed by SPSS13 .0 .RESULTS During the research, 529 out of 27 315 investigated inpatients suffered hospital acquired infection .The prevalence rate was 1 .94% .For medicine system, the departments with top three infection rates were the department of hematology and implantology (13 .68% ), ICU (8 .48% ) and respiration (3 .90% ). For surgery system, the departments with top three infection rates were the department of burn (5 .3% ), thoracic surgery (3 .04% ) and vascular surgery (2 .78% ). The main infection site was respiratory tract, accounting for 35 .16%, followed by urinary system (23 .25% ) and blood system (19 .85% ) .Among the detected pathogens, gram-positive bacteria accounted for 45 .77%, gram-negative bacteria accounted for 31 .51% and fungi for 22 .72% .CONCLUSION The effective implementation of measures to control infections, strengthening the supervision to the key departments and key crowd, reducing invasive process, using antibiotics rationally, and controlling by multiple angles, factors and links should be carried out to reduce the rate of hospital infections .
