摘要 目的评价CellaVision DM96全自动血细胞形态分析仪(简称DM96)对外周血白细胞与非白细胞成分识别与分类的性能。方法选取来自健康体检人员、肿瘤患者及白血病患者共155例外周血标本,经涂片染色,用DM96对外周血中白细胞与干扰白细胞识别的非白细胞成分共17类进行预分类,经专业人员审核再分类,以再分类结果作为"金标准"计算DM96对这17类成分分类的敏感性、特异性、阳性预测值和阴性预测值。结果本研究DM96共采集白细胞与非白细胞成分19 587个,总正确率为82.3%(16 123/19 587)。DM96在健康体检人员和肿瘤患者组中对中性粒细胞、淋巴细胞预分类的敏感性和阳性预测值均>85%。DM96对检测幼稚粒细胞的敏感性及阳性预测值均<50%,对原幼细胞的敏感性较低,为36.1%(223/618),但阳性预测值良好,为92.1%(223/242)。结论 DM96对成熟细胞有较高识别能力,但对幼稚粒细胞和原幼细胞的识别不佳;可作为外周血涂片形态学复检的辅助工具,但对含有幼稚细胞较多的白血病外周血标本,建议仍进行人工显微镜检查。 Objective To assess the performance of CellaVision DM96 system (DM96) in the analysis for leukocytes and non-leukocytes in peripheral blood cells. Methods A total of 155 peripheral blood samples from healthy donor, tumor patients and leukemia patients were col- lected and prepared by automated smear and stain system. All the slides were analyzed by DM96 system. The blood cells were pre-classified into 17 categories including leukocytes and non-leukocytes. The results of DM96 image were then reviewed by medical technologist and the cells were reclassified if necessary. The sensitivity, specificity, positive predictive value and negative predictive value of all the 17 categories in DM96 system were evaluated. Results In total, 19 587 leukocyte or non-leukocyte components were classified by DM96 getting a correct rate of 82.3% ( 16 123/19 58"7). Both the sensitivity and positive predictive value of mature granulocytes and lymphocytes from healthy donor and tumor patients in DM96 were greater than 85%, while both the sensitivity and positive predictive value of immature granulocytes in DM96 were less than 50%, The sensitivity for blasts was only 36. 1% (223/618), but the positive predictive value was still high (92. 1%, 223/ 242). Conclusion The DM96 system showed high recognition for mature white blood cells, but poor identification for inrnature granulocytes and blasts. DM96 can he used as an auxiliary instrument for reviewing the morphology of white blood cell, but manual microscope examina- tion is still necessary for leukemia samples with large amounts of immature granulocytes and blasts.
