摘要
目的：评价经乳晕缘切口乳腺良性肿瘤切除术的临床疗效。方法根据患者自愿和肿瘤情况，将患者分为乳晕缘切口组和肿瘤原位切口组。其中乳晕缘切口组沿乳晕缘行切口，肿瘤原位切口组沿肿瘤原位行切口。记录两组术后瘢痕评分（ VSS）、术后满意度，并观察患者乳晕区异常状况。结果乳晕缘切口组VSS评分为（1.72±1.04）分，肿瘤原位切口组VSS评分为（5.44±1.82）分，差异有统计学意义（t=16.275，P﹤0.01）。乳晕缘切口组手术满意率为96.88%，肿瘤原位切口组为73.68%，差异有统计学意义（ P﹤0.01）。乳晕缘切口组发生乳晕区异常13例（20.31%），切口长度越长，患者乳晕区异常的发生风险越高。结论经乳晕缘切口行乳腺良性肿瘤切除术可以有效改善患者的VSS评分，提高患者对手术的满意度，是一种可以较好满足爱美女性需求的手术方法，值得临床应用。但为了避免乳晕区感觉异常，应尽量控制切口长度在1/2以内。
Objective To evaluate the clinical efficacy of areola incision surgery on benign breast tumor. Meth-ods According to the voluntary principle and tumor situation,the patients were divided into areola incision group and tumor in situ incision group. The incision was along the edge of the areola in areola incision group,and the incision was along in situ tumor in situ incision group. The postoperative scar score（ VSS）,postoperative satisfaction degree was recorded,and the areola area abnormalities were observed. Results The VSS score in areola incision group was 1. 72 ± 1. 04, and 5. 44 ± 1. 82 in tumor in situ incision group. There was significant difference in VSS score between the two groups（ t=16. 275,P﹤0. 01）. The operation satisfactory rate was 96. 88% in areola incision group,and 73. 68% in tumor in situ incision group. There was significant difference in the operation satisfactory rate between the two groups（P﹤0. 01）. Areola incision group had areola area abnormalities in 13 cases（20. 31%）,the longer the length of the incision,the higher the incidences of areola area abnormalities were. Conclusions Benign breast tumor by areola incision can effectively improve the patient’s VSS score,improve the satisfaction degree of the surgery,is a surgical method which can better meet the needs of female beauty,so is worthy of clinical application. However,in order to avoid the areola area paresthesia,the incision length should be controlled in less than 1/2 as far as possible.
