摘要
目的探讨低位房间隔(LAS)起搏的可行性及方法学。方法选择45例符合起搏器植入标准的患者,随机分成二组。一组采用主动固定螺旋电极行LAS起搏(LAS组),另一组采用被动固定翼状电极行右心耳(RAA)起搏(RAA组)。分别记录两组心房电极操作时间、X线曝光时间、以及所测试的起搏阈值和阻抗。术后1、3个月门诊随访常规心电图、测量起搏参数和X线胸片观察电极脱位情况。结果与RAA组比较,LAS组心房电极操作时间和X线曝光时间无显著差异[(5.6±0.9)min vs(5.2±1.0)min,(4.4±0.7)min vs(4.0±0.9)min,P均>0.05]。两组术中,术后1、3个月起搏参数均无显著差异(P>0.05)。结论主动固定螺旋电极行LAS起搏操作简单易行,且不影响起搏阈值和阻抗。
Objective To explore the feasibility and methods of low atrial septal pacing. Methods Forty-five pa- tients with pacemaker implantation standard, were randomly divided into two groups. A group with low atrial septal pacing by active fixation electrode (LAS group), another group with traditional right atrial appendage pacing by electrode passive fixation (RAA group). The two groups were recorded in atrial electrode operating time, X-ray exposure time, and the pa- cing threshold and impedance. In first months, third months after operation, electrocardiogram was recorded, the pacing pa- rameter was measured and electrode dislocation was observed by X-ray. Results Atrial electrode operation time and ex- posure time of X-ray had no significant difference between two groups. Pacing parameters in first month, third month at postoperative were also not statistically in two group [ (5.6±0.9) min vs (5.2±1.0) min, (4.4±0.7) min vs (4.0±0.9) min, all P〉0.05 ]. Conclusion Low atrial septal pacing through active fixation electrode is safe and don't effect pacing paramenters.
