摘要
目的采用Meta分析方法分析阿司匹林、氯吡格雷及两者联合应用抗血小板作用的效果。方法采用Meta分析方法,检索MEDLINE、EMbase、Cochrance临床对照试验资料库、中国生物医学文献数据库、中国学术期刊全文数据库、维普数据库中相关文献,收集所有关于以血栓弹力图评价阿司匹林及氯吡格雷抗血小板作用的研究文献。应用RevMan5.2软件对阿司匹林及氯吡格雷抗血小板作用进行分析。结果共纳入5篇病例对照研究,其中包括阿司匹林组206例,氯吡格雷组205例,联合应用组300例。氯吡格雷组二磷酸腺苷(ADP)途径血小板抑制率低于阿司匹林组花生四烯酸(AA)途径的抑制率(P<0.05);阿司匹林组AA途径诱导的血小板抑制率显著低于联合应用组(P<0.05);氯吡格雷组ADP诱导的血小板抑制率显著低于联合应用组(P<0.05)。结论常规剂量阿司匹林抗血小板作用优于常规剂量氯吡格雷,阿司匹林与氯吡格雷联合应用抗血小板具有协同作用。
Objective To analyze the differences in the antiplatelet effects of aspirin , clopidogrel and joint application by Meta-analysis.Methods According to Meat-analysis, MEDLINE, EMbase, CCTR, CBMdisc, CNKI and VIP database were searched for the related randomized controlled trials (RCTs).Analysis was performed by RevMan5.2 software to study the antiplatelet effects of aspirin and clopidogrel .Results Five case-control studies were included , containing 206 cases in the aspirin group , 205 cases in the clopidogrel group and 300 cases in dual-therapy group .The platelet inhibition rate of clopidogrel group ADP ( Adenosine diphosphate ) pathway was significantly lower than that of aspirin group AA (Arachidonic acid) pathway (P〈0.05); the platelet inhibition rate of aspirin group AA pathway was significantly lower than that of dual-therapy group (P〈0.05);the platelet inhibition rate of clopidogrel group ADP pathway was significantly lower than that of dual-therapy group (P〈0.05).Conclusions The antiplatelet effect of regular dose of aspirin is better than that of clopidogrel .Aspirin and clopidogrel have synergistic antiplatelet effect .
