摘要
目的:评价涎腺内镜在慢性颌下腺炎病因诊断中的价值。方法:对78例因颌下腺反复肿胀就诊的患者行颌下腺涎腺内镜检查,明确导管系统的病变。结果:7例因导管重度狭窄放弃检查,71例完成涎腺内镜检查(91.0%)。59例通过涎腺内镜可发现明确的导管系统病变,其中结石31例,23例存在导管增生扩张、粘液栓及纤维素样渗出形成;导管息肉2例,导管畸形2例。多数病例导管内壁可见不同程度的充血、糜烂性炎症。此外,12例患者通过内镜未发现可观察到的明确的病变。结论:涎腺内镜可直观、有效地观察颌下腺导管系统的病变,具有较高的诊断价值。
Objective：To evaluate the value of sialodenoscopy for etiologic diagnosis of chronic submandibular sialadenitis. Method：Seventy-eight patients presenting recurrent swelling of the submandibular gland were detected with sialodenoscopy. The features of the ductal system were observed by diagnostic endoscopy. Result：Seventy-one patients were accomplished the detection of sialodenoscopy （91.0%）,and the remaining 7 patients gave up the detection because of seri-ous duct stenoses. The definite intraductal diseases were observed by the endoscopy in 59 patients , including sialoliths（31/78）,ductal strictures and expansion, mucus plug and fibrinoid exudation,polyps （2/78） and ductal malformations（2/78）. Inflammations on different degrees were found in the vast majority of above cases. In addition ,no obvious changes were detected by endoscopy in 12 patients. Conclusion：This study demonstrates sialodenoscopy as a promising new method has a high feasibility and effectiveness to diagnose the etiology of chronic submandibular sialadenitis.
