摘要
目的 探讨手足口病合并脑炎患儿免疫球蛋白和补体的变化及临床意义.方法 选取2012年3月～2013年8月在本院儿科住院的120例手足口病患儿,其中60例为手足口病普通病例（第1组）,60例为手足口病合并脑炎重症病例（第2组）,患儿于入院当日治疗前采集静脉血作血清免疫球蛋白及补体检测,取同期42例门诊体检健康儿童为第3组,比较3组的血清免疫球蛋白IgA、IgG、IgM及补体C3、C4的差异.结果 第1组和第2组IgA水平较第3组显著下降（P＜0.05）;第2组IgG水平较第3组明显降低（P＜0.05）;第1组和第2组的IgM及补体C3、C4水平与第3组比较,差异无统计学意义（P＞0.05）.结论 手足口病并脑炎患儿IgA、IgG水平下降,为临床进行免疫治疗提供了重要依据.
Objective To investigate the changes and clinical significance of immunoglobulin and complement in children with hand-foot-mouth disease associated with cerebritis. Methods 120 cases of children with hand-foot-mouth disease treated in our hospital from March 2012 to August 2013 were selected,of whom,there were 60 common patients with hand-foot-mouth disease （group 1） and 60 severe patients with hand-foot-mouth disease associated with cerebritis （group 2）,all patients were received serum immunoglobulin and complement tests on the day of hospital admission.42 healthy children receiving physical examination in the outpatient department in the same period were selected as group 3.The differences between the 3 groups in serum immunoglobulin IgA,IgG and IgM and complement C3 and C4 were compared. Results The IgA levels of group 1 and group 2 reduced significantly compared with that of group 3,with significant difference （P〈0.05）;The IgG level of group 2 reduced obviously compared with that of group 3,with significant difference （P〈0.05）;The IgM and complement C3 and C4 levels of group 1 and group 2 had no significant difference compared with those of group 3 （P〉0.05）. Conclusion The decreasing of IgA and IgG levels of children with hand- foot-mouth disease associated with cerebritis provides important reference for clinical immunotherapy.
