摘要
目的探讨后腹腔镜肾切除术治疗重度积水无功能肾的可行性和安全性。方法分析我院2007年1月至2013年12月因重度积水无功能肾而进行肾切除术的患者46例,其中后腹腔镜肾切除术组(retroperitoneal laparoscopic nephrectomy)的22例为后腹腔镜(RN)组,开放肾切除术组(open nephrectomy)的24例为开放(ON)组。比较两组围手术期的相关指标。结果患者平均手术时间RN组[(1293±20.5)min]与ON组[(118.2±19.3)min]比较差异无统计学意义(P〉0.05)术中出血量RN组[(162.0±22.1)m1]少于ON组[(259.2±18.5)m1],两组比较差异有统计学意义(P〈0.01);切口长度RN组[(47.3±19.4)mm]少于ON组[(151.2±25.2)mm],两组比较差异有统计学意义(P〈0.01);RN组肠功能恢复时间[(25.6±4.9)h]及住院时间[(8.1±1.8)d]均少于ON组(P〈0.01);RN组术后并发症率(9.1%)低于ON组(33.3%)(P〈0.05)。结论后腹腔镜肾切除术治疗重度积水无功能肾安全有效。
Objective To investigate the feasibility and safety of retroperitoneal laparoscopic nephrectomy in the treatment of nonfunctioning kidney with severe hydronephrosis. Methods 46 cases of nonfunctioning kidney with severe hydronephrosis in our hospital from January 2007 to December 2013 were selected, 22 cases in whom received retroperitoneal laparoscopic nephrectomy (RN group), 24 cases received open nephrectomy (ON group). Pefioperative indexes of two groups were observed and analyzed. Results There was no statistically significant difference in average operation time between RN group and ON group [(129.3 ± 20.5) min vs.(118.2 ± 19.3)min, P 〉 0.05]. The intraoperative blood loss of RN group was less than that of ON group [(162.0 ± 22.1)ml vs.(259.2 ± 18.5)ml], with statistically significant difference (P 〈 0.01). The incision length of RN group was less than that of ON group [(47.3 ± 19.4)mm vs.(151.2 ± 25.2)mm], with statistically significant difference (P 〈 0.01). The intestinal function recovery time [(25.6 ± 4.9)h] and hospital stay [(8.1 ± 1.8)d] of RN group were less than those of ON group (P 〈 0.01). The postoperative complication rate of RN group was lower than that of ON group (9.1% vs.33.3%; P 〈 0.05). Conclusion Retroperitoneal laparoscopic nephrectomy in the treatment of nonfunctioning kidney with severe hydronephrosis is feasible and safe.
