摘要
[目的]探讨社区护士参与高血压慢性病管理的有效模式。[方法]选择同一街道两个居委会中确诊为原发性高血压并达到中度危险层的病人为研究对象,将符合入选标准的研究对象分为两组,对照组采用常规管理方法,观察组根据《中国高血压防治指南》内容制定临床路径(护士版、医生版、病人版),按临床路径对高血压病人实施慢性病管理,观察两组病人高血压相关知识达标率、血压控制率和管理满意度。[结果]两组病人高血压健康教育知识达标率、血压控制率、病人满意度比较差异有统计学意义(P<0.05)。[结论]运用临床路径对社区高血压慢性病进行管理,能提高健康教育知识达标率、血压控制率,同时也提高了社区护理工作满意度及社区护士的执行力。
Objective:To explore an effective mode of community nurses participated chronic disease management for patients with hypertension.Methods:To choose confirmedly diagnosed primary hypertension of moderate risk layer patients in the same street of two residents committee as research subject.Among them,those meet the inclusion criteria were divided into two groups.Patients in control group adopted the conventional management method.And patients in observation group were given chronic disease management for patients with hypertension based on the clinical pathway(nurse version,doctor version,and patient version)worked out according to the contents of'Chinese hypertension prevention and treatment guideline'.Then up to the standard rate of hypertensive related knowledge,blood pressure control rate and management satisfaction of two group cases were observed.Results:There were statistical significant differences in terms of up to the standard rate of hypertension knowledge of health education,blood pressure control rate and management satisfaction between the two groups(P<0.05).Conclusion To apply clinical pathways in management of hypertension chronic disease in community can enhance up to the standard rate of health education knowledge and control the rate of blood pressure of hypertension patients.And meanwhile it can raise the job satisfaction of community nursing and performance ability of community nurses.
