摘要
目的探讨乳腺黏液囊肿样病变（MLL）的临床病理特点及其诊断方法。方法复习长兴县人民医院病理科1997年1月—2008年2月乳腺外检切片,其中被诊断为MLL的有5例,采用随机数字表法分别编号为1、2、3、4、5。采用阿利新兰-过碘酸雪夫（AB-PAS）染色法进行组织化学染色,对AE1/AE3、c-erbB-2、p63采用SP法进行免疫组织化学染色。结果 5例MLL患者均为女性,年龄24~47岁,平均36.6岁,随访10~109个月,无腋下淋巴结转移,无复发。单侧乳房可触及肿块,肿物切面呈多囊性黏稠胶胨样。镜下观察可发现多发性的囊肿,囊肿内充满黏液。囊内存在扁平、柱状或立方上皮,5例中上皮有乳头状增生3例、不典型增生1例。间质内有无内衬上皮的黏液湖。间质和内囊内的黏液湖经AB-PAS染色均呈阳性。囊肿上皮细胞和其黏液湖内的腺管上皮细胞AE1/AE3和肌上皮细胞p63经SP染色均呈阳性,而c-erbB-2经SP染色呈阴性。结论乳腺MLL为一种独立的良性乳腺增生性的瘤样病变,易误诊为乳腺黏液腺癌,故建议诊断时要注意将乳腺黏液腺癌等肿瘤与MLL区分开,并对患者定期随访。
Objective To study the clinical and pathological features of breast mucocele- like lesions（ MLL）.Methods Five MLL mammary gland slices in department pathology,Changxing County People’s Hospital from January 1997 to February 2008 were numbered randomly into No. 1,2,3,4,5. Histochemical stain was performed by Alcian blue- periodic acid sthiff（ AB- PAS）,and streptavidin- perosidase（ SP） method used to do immunohistochemical staining on immunologic tissues of AE1 /AE3,c- erbB- 2,p63. Results The 5 MLL patients were all female,aged 24- 47 years,meanly 36. 6years old. Follow- up lasted 10 to 109 months. No axillary lymph node metastasis occurred,or reoccurred. Tumors were touched in unilateral breast,the cut surface of tumor was polycystic viscous- jelly- like. Microscope found multiple cysts filled with mucus. In cysts were flat,columnar or cubic epithelium cells. In 5 patients,3 had papillary epithelial hyperplasia,1 had atypical hyperplasia. In mesenchymal was mucus lake of non- lining epithelium. The mesenchymal and mucus lake were positive after AB- PAS staining. Cryptae epithelial cell AE1 /AE3 and myoepithelial cell p63 in cyst epithelial cell and its mucus lake were positive after SP staining,c- erbB- 2 was negative. Conclusion Breast MLL,an independent benign mammary hyperplasia tumor- like lesion,is easy to be misdiagnosed as breast mucinous adenocarcinoma.
