摘要
目的探讨健康信念模式与自我效能理论在髋关节置换术患者康复训练干预中的效果。方法将100例髋关节置换术后患者随机分为干预组与对照组,每组50例。对照组按髋关节置换术常规功能康复锻炼流程,让患者学习掌握功能锻炼的内容、方法和时间。干预组在此基础上,运用健康信念模式及自我效能理论设计干预方案进行干预。对干预前、后不同时间干预组与对照组患者康复训练的依从性、康复锻炼疼痛程度、髋关节功能、焦虑及抑郁程度进行比较分析。结果干预组患者术后康复训练依从性等级、自我效能评分、术后2周及3个月Harris评分均明显高于对照组,差异有统计学意义(P<0.05)。结论健康信念模式及自我效能理论干预可提高髋关节置换术后患者功能锻炼的依从性,对调动患者自身潜能,促进功能恢复具有肯定的疗效。
Objective To evaluate the clinical effect of health belief model and the self - efficacy theory in rehabilitation of patients with hip replacement . Methods FromApril 2013 to December 2013, 100 patients with hip replacement were evenly randomized into two groups ： test group （ n = 50） and control group （ n = 50）. There were 50 males and 50 females, aged from 51 - 75 years （ average 68.75 ± 6.65 years ） . Both groups undertook rehabilitative exercises as directed by doctors. The control group was given routine treatment and health instruc- tion , while the test group received health belief model and promoting self - efficacy intervention besides the routine treatment care. The compli- ance of rehabilitative exercises, pain intensity during the exercises , the function of hip joint in both groups were compared , and their anxiety and depression scale was measured with Zung Self -rating Anxiety Scale and Self- rating Depression Scale . Results All patients were fol- lowed up . Comparing with the control group,the compliance of rehabilitative exercises, self- efficacy score , the function of hip joint, anxiety and depression status were markedly improved,the pain during exercises was obviously relieved in the test group. Conclusion Health belief model and self - efficacy theory can improve patients＇ compliance of rehabilitative exercises , promote the patients potentials and improve their rehabilitation and quality of life.
