摘要
目的:通过对2型糖尿病以及糖尿病前期合并阻塞型睡眠呼吸暂停综合征(OSAS)患者的中医问卷调查,探讨此类患者的中医证候及相关特点。方法:通过糖耐量试验和多导睡眠监测筛选出103例研究对象,将其分为4种中医证候类型,分析证型比例以及各证型之间年龄、BMI、呼吸暂停低通气指数(AHI)、夜间最低血氧饱和度(SaO2low)的差异。结果:痰热蕴肺型占总体人数的48.5%,其他3型所占比例13.6%-20.4%不等;年龄上,痰热蕴肺型小于脾虚痰阻型和肺肾两虚型,痰瘀互结型小于肺肾两虚型;BMI指数,肺肾两虚型低于痰热蕴肺型和痰瘀互结型,脾虚痰阻型低于痰瘀互结型;痰热蕴肺型的AHI高于其他3型,SaO2low低于其他3型,以上差异均有统计学意义(P<0.05,P<0.01)。结论:痰热蕴肺型是该类患者的主要中医证型,痰热蕴肺型的OSAS病情程度要重于其他3型,随着患者年龄的增大,正气逐步亏虚,形体趋于消瘦。
Objective: To explore the TCM syndrome characteristics of 103 cases patients with type 2 diabetes mellitus (T2DM) and pre-diabetes combined with obstructive sleep apnea syndrome (OSAS) by the questionnaires. Methods: 103 objects were selected by oral glucose tolerance test (OGTT) and polysomnography (PSG), who were divided into 4 TCM syndromes types. Then, the proportion of TCM syndromes and the difference among 4 TCM syndromes were analyzed. Results: The syndrome of phlegm-heat accumulated in the lung (PHAL) accounted for 48.5% of the total, while the other 3 types accounted for range from 13.6% to 20.4%; The average age of patients with PHAL was younger than that with the syndrome of spleen-deficiency and phlegm-stagnation (SDPS) and that with the syndrome of both lung and kidney deficiency (BLKD), and the average age of patientswith the syndrome of phlegm and blood stasis (BPBS) was younger than that with BLKD; The body mass index (BMI) of patients with BLKD was lower than that with PHAL and that with BPBS, and the BMI of patients with SDPS was lower than that with BPBS; The apnea-hypopnea index (AHI) of patients with PHAL was higher than the other 3 types, and the SaO2low of patients with PHAL was lower than the other 3 types. There were statistical significant difference in above data (P〈0.05, P〈0.01). Conclusion: PHAL is the main TCM syndrome in T2DM and pre-diabetes combined with OSAS. The OSAS severity in PHAL is higher than the other 3 types. As the older patients grow and the less vital qi is consumed, the thinner patients get.
