摘要
目的探讨颈椎动态MRI结合临床表现对早期脊髓型颈椎病（CSM）的诊断价值。方法将研究对象分为3组,均为2012年12月~2013年12月来我院就诊患者,亚健康组（A组）35例,该组患者主诉颈肩部不适但非颈椎病;B组35例,该组患者的临床症状、体征与CSM符合但常规MRI结果无异常;CSM组（C组）35例,该组患者为确诊CSM。105例患者均进行X线片及MRI检查,用统计学方法比较结果。结果 B组及CSM组JOA评分、X线结果中前凸角、椎管矢径、椎体/椎管矢径比,中立位、过伸位及过屈位MRI椎管矢径与亚健康组比明显较低,而比率明显较高,差异有统计学意义（P〈0.05）。B组常规颈段MRI及动态颈段MRI检查中出现脊髓受压呈念珠样改变的例数比较差异有统计学意义（P〈0.05）。结论在早期脊髓型颈椎病的诊断中颈椎动态MRI具有重要的价值,优于常规MRI。
Objective To investigate the diagnostic value of cervical vertebra dynamic MRI combined with clinical manifestations for early cervical spondylotic myelopathy. Methods 105 cases selected from Dec 2012 to Dec 2013 in our hospital were divided into three groups. The patients in sub-healthy group （35 cases） had complained of neck and shoulder discomfort, but did not have cervical disease; the patients B group（35 cases） had clinical symptoms and signs in line with CSM but conventional MRI showed no abnormalities; the patients in CSM group （35 cases） were diagnosed with CSM. 105 cases under- went X-ray and MRI, the results was compared by statistical methods. Results JOA score, lordosis, spinal sagittal diameter, vertebral canal sagittal diameter ratio in X-ray results, the diameters of the cervical cord in neutral position, flexion and extension position in B group and CSM group were significantly lower than those of the sub-healthy group, while the ratio was significandy higher, the difference was significant（P〈0.05 ）. There was significant difference in the number of cervical spinal cord compression rosary-like changes in B group between routine cervical MRI and dynamic MRI （P〈0.05）. Conclusion Cervical vertebra dynamic MRI has important value in the diagnosis of early cervical spondylotic myelopathy, it is superior to conventional MRI.
