白细胞精液症治疗前后精浆瘦素与精液常规、精子凋亡率的检测及相关性分析 被引量:1

Test and correlation analysis of seminal Leptin, sperm parameters and apoptosis in leukocytospermia patients

摘要 目的:探索白细胞精液症患者治疗前后精液常规、精浆瘦素(Leptin)及精子凋亡率与正常人群的差异,并分析其相关性。方法随机选取2010年10月至2012年5月本院泌尿男科门诊的白细胞精液症患者80例(WBC组),另选取生育力及各项精液参数均正常的生育人群标本40份作为对照组,在患者治疗前及经治疗精液白细胞水平恢复正常后,采用计算机辅助精液分析系统(CASA)行精液常规检测、放射免疫法检测精浆Leptin、Annexin V+PI染色-流式计数法测定精子凋亡率。比较各参数的组间差异并分析其相关性。结果 WBC组治疗前精子浓度及活力均低于对照组(P<0.05,P<0.01),精子凋亡率显著高于对照组(P<0.01),精浆Leptin水平高于对照组(P<0.05),精浆Leptin水平与精子凋亡率之间存在正相关性(r=0.678,P<0.05),经治疗后精子活力显著增加(P<0.01),凋亡率降低(P<0.05),精浆Leptin水平亦明显下降(P<0.05)。结论治疗前白细胞精液症患者精子浓度、活力下降,而精浆Leptin水平和凋亡率均有增加,精浆Leptin与精子凋亡率之间存在正相关性。治疗后精浆Leptin与精子凋亡率均出现下调,精液常规各项参数明显改善,提示Leptin可能参与了过量白细胞诱发精子细胞凋亡的途径。 Objective To detect the seminal Leptin level, sperm parameters and sperm apoptosis rate of leukocytospermia patients before and after treatment, and explore their correlation. Methods Total of 80 leukocytospermia patients(WBC group)were randomly recruited in the study and their sperm samples were collected before and after treatment. Other 40 normal sperm samples were collected as the controls. The sperm parameters were detected by computer assisted sperm analysis (CASA). The level of semen plasma Leptin was measured by RIA and the sperm apoptosis rate were assessed by AnnexinV/PI staining-FACS. Results Sperm concentration and viability in WBC group before treatment were all obviously lower than those of the control (P〈0.05,P〈0.01), whereas the apoptosis rate was significantly higher(P〈0.01). The level of semen plasma Leptin was higher than that of the control (P〈0.05), moreover itwas positively correlated with sperm apoptosis rate (r=0.678,P〈0.05). After treatment, semen activity was increased and sperm apoptosis rate and Leptin level were decreased in WBC group. Conclusion Increase inseminal Leptin level and sperm apoptosis rate were founded in WBC group, which may be contributed to the decrease of sperm concentration and viability. Positive relationship between seminal Leptin and sperm apoptosis was found, implyingthat Leptin might play a role in sperm apoptosis by increasing seminal leukocyte.
