摘要 目的探讨皮肤来源细胞移植治疗稳定期白癜风的有效性,以及与皮肤移植等其他治疗的效果比较。方法检索Pubmed,EMbase,Cochrane library,CNKI,cBMDisc等数据库。纳入研究为皮肤来源细胞移植治疗稳定期白癜风的临床对照研究。采用随机效应模型,危险度比描述合并值。结果最终纳入文献10篇,其中细胞治疗组与空白对照组相比效果差异有统计学意义(P<0.000 1);细胞治疗组与皮肤移植治疗效果相比,差异无统计学意义(P=0.39);经培养细胞治疗与未经培养细胞治疗效果相比,差异无统计学意义(P=0.17)。结论皮肤来源细胞治疗难治性稳定期白癜风有一定的效果。但在细胞移植与皮肤组织移植治疗之间仍无明显的差异。另外,细胞扩增对治疗效果不会产生明显影响。 Objective To discuss and compare the therapeutic effect of skin-derived cells transplantation with other treatment on stable vitiligo. Methods The reference lists of articles in Pubmed, EMbase,Cochrane library, CNKI and cBMDisc were searched. The controlled trials which reported the effects of skin derived cell on vitiligo were included for meta-analysis. Relative risk was used to measure the effect on vitiligo under random effect model. Results Ten trials were included in. The skin cell transplantation had a significant therapeutic effect on stable vitiligo compared with placebo or no treatment (P 〈 0. 000 1 ). But there were no statistical different of therapeutic effect between the cell treatment and skin tissue treatment ( P = 0. 39), and expanded and non-expanded cell treatment (P = 0. 17 ). Conclusion The skin derived cells have some therapeutic effect on stable vitiligo. The therapeutic effect is no different between the cell transplantation and skin transplantation. Whether expand skin derived cells would be no change the therapeutic effect.
