摘要
目的:探讨关节镜下同种异体肌腱胫骨、股骨独立隧道解剖止点中点单束重建前交叉韧带(anterior cruciate ligament,ACL)的近期效果。方法:2009年9月~2011年9月,收治33例陈旧性ACL断裂患者。在关节镜下进行单束重建,通过胫骨、股骨独立建立隧道将移植同种异体肌腱定位于胫骨、股骨止点的中点并固定。应用Lachman和轴移实验及KT-1000评估膝关节稳定性,按照国际膝关节评分委员会评分标准(IKDC)、Lysholm膝关节评分评价膝关节功能。结果:患者均获随访,随访时间13~24个月,平均16.6个月。术后患者切口均Ⅰ期愈合,无关节内感染、下肢深静脉血栓形成和血管、神经损伤等并发症发生。均未发现免疫排异反应及过敏反应。术后1年随访结果:术后Lachman试验、轴移试验、KT-1000检查、IKDC膝关节功能评分、Lysholm膝关节评分与术前比较差异均有统计学意义(P<0.05)。结论:关节镜下利用胫骨、股骨独立隧道解剖止点中点同种异体肌腱单束重建ACL可恢复膝关节稳定性,近期疗效较好。
Objective In this study,arthroscopic single-bundle ACL reconstruction with allogenic tendon positioning at the insertion midpoint of tibia and femur was retrospectively analyzed. Methods Thirty-three patients (18 males and 15 females with an average age of 27.4 years,ranging from 22-33 years)with chronic ACL ruptures were reconstructed arthroscopically in our hospital from September 2009 to September 2011. The cause of injury included traffic accident(15/33),falling down(14/33),and sports trauma (4/33). The average duration from injury to surgery was 6 months (ranging from 2 to 20 months). The single-bundle grafts were positioned at midpoint of original ligament insertion of tibia and femur with independent tibial and femoral tunnels. KT-1000 arthrometry measurement,Lachman and pivot-shift test, knee function of the International Knee Documentation Committee (IKDC), and Lysholm scores were used for clinical evaluation. Results All the patients were followed up for an average of 16.6 months (ranging 13 to 24 months). No serious complications (such as injuries of artery or nerves, infection of articulation,and deep venous thrombosis)were found. The results of KT-1000 arthrometry measurement, Lachman and pivot-shift test, knee function of IKDC, and Lysholm scores after one year surgery improved significantly (P 〈 0.05)in all 33 patients. Conclusion Arthroscopic single-bundle ACLreconstruction with allogenic tendon positioning at the insertion midpoint of tibia and femur yielded satisfaction clinical outcome.
