摘要
目的:评价计算机辅助检测系统(CAD)在乳腺单纯性钙化病灶中的应用价值及其对诊断医师的影响。方法:46例经全视野数字化乳腺X线摄影(FFDM)检出的单纯性钙化灶患者,包括单纯中间性钙化灶和单纯恶性钙化灶,比较CAD分析前后每个病灶的BI-RADS分类有无差异。比较放射医师在CAD分析前后诊断病灶的准确性和检出恶性病灶的敏感性有无差异。结果:放射医师对单纯钙化性病灶的BI-RADS分类评价在使用CAD前后差异有显著性意义(χ2=78.04,P<0.001),使用CAD前后的诊断准确性差异有显著性意义(P=0.001),检出恶性病变的敏感性在使用CAD前后差异无显著性意义(P=0.059)。结论:在临床患者人群中使用CAD能够帮助放射医生提高对乳腺单纯性钙化灶的诊断准确性,但对单纯恶性钙化病灶的诊断敏感性没有明显帮助。
Objective:The purpose of this study was to evaluate the impact of computer-aided detection (CAD) on the performance of mammogram reader in interpreting pure calcification lesion. Methods: A retrospective review was performed in 46 patients with pure calcification lesion detected by full-field digital mammography (FFDM) ,and every lesion was given a BI-RADS score before and after CAD analysis. The BI-RADS scores of lesion before and after CAD analysis were com- pared. The overall diagnostic accuracy and sensitivity for detecting malignant lesion were compared between analysis before and after CAD. Results: There was significant difference in the BI-RADS scores (x2= 78.04 ,P〈0. 001) and in the diagnos- tic accuracy of lesions (P=O. 001) between analysis before and after CAD. There was no significant difference in the sensi tivity of detecting malignant lesion between analysis before and after CAD (P= 0. 059). Conclusion:Our results indicate that systematic application of CAD with FFDM in a clinical patient population should be effective in assisting radiologist to im- prove the mammogram interpretation accuracy and the sensitivity of detecting breast cancer.
