摘要
目的 分析幽门螺杆菌(Helicobacter pylori,Hp DNA的cagA,vacA和iceA三种毒力基因及其多重组合与消化性疾病之间的关系.方法 2013年6月-11月收集复旦大学附属华东医院消化内镜室的快速尿素酶检测阳性的胃镜活检标本108例,直接进行DNA抽提,利用GeXP多重基因分析系统检测3种毒力基因.结果 108例患者中慢性胃炎56例,消化性溃疡39例,胃癌13例.108例胃活检标本中83例(76.9%)检出cagA基因(cagA+),其中42.2% (35/83)为消化性溃疡患者,cagA+菌株与消化性溃疡之间的关系差异有统计学意义(P=0.038).76.9%(30/39)消化性溃疡患者标本包含s1基因型,79.5% (31/39)包含m1基因型,s1和m1基因型与消化性溃疡之间的关系差异有统计学意义(P=0.049,0.007); 69.2% (27/39)包含s1/m1基因型,12.8% (5/39)包含s2/m2基因型,s1/m1组合与消化性溃疡之间的关系差异有统计学意义(P=0.016).基因型组合cagA+/s1/m1与消化性溃疡密切相关(P=0.043);cagA +/vacAs1m1/iceA1也与消化性溃疡相关(P=0.025).结论 Hp的基因cagA、vacA、s1/m1、iceA1及其组合与消化性溃疡密切相关.
Objective To investigate the relationship among Helicobacter pylori (Hp) virulence genes (cagA, vacA, iceA) by analyzing DNA extracted from gastric biopsies specimens directly. Methods We collected 108 Hp positive specimens from Huadong Hospital endoscopy department during June to November in 2013. GeXP was used to analyse Hp virulence genes. Results 83 cases (76.9%) contained cagA gene, and the relationship between cagA and peptic ulcer was significant (P=0.038). 30cases (76.9%) contained s1 genotype and 31cases (79.5%) contained m1 genotype, sl and ml were associated with peptic ulcer (P = 0.049 and 0.007 ). In the peptic ulcer group, 27 (69.2%) cases included s1/m1 genetype, and only 5 cases (12.8%) included s2/m2 genetype, s1/m1 was associated with peptic ulcer (P= 0.003). cagA+ genotype was always associated with s1/m1 genotype, and was closely related to peptic ulcer (P= 0.016). Both of the genotype combination cagA+/s1/m1 and cagA+/s1/m1/iceA1 were related to peptic ulcer (P = 0.043 and 0.025). Conclusions cagA, vacA s1/m1 and their combination existed in Hp were closely associated with peptic ulcer.
