婴幼儿毛细血管瘤与毛细血管畸形的非线性光谱成像研究

摘要 目的探讨非线性光谱成像技术在临床上用于人活体内毛细血管瘤及毛细血管畸形的实时、无创性分析及诊断的潜在价值和可行性。方法选取我院2012年9月至2013年11月收治的36例婴幼儿颌面部皮肤毛细血管瘤(CH)及毛细血管畸形(CM)手术切除标本,其中CH(2~12个月)22例,CM(4~12个月)14例,相应的正常皮肤(NS)为对照组36例。将所取的样本按30μm厚度切割制片,所有样品用非线性光谱成像系统进行分析。结果在同一成像深度,最佳激发波长下,正常皮肤与病变组织中胶原蛋白的含量存在差异;CH中弹性纤维直径分别明显低于CM及NS,差异均有统计学意义(P〈0.05,P〈0.01),CM与NS比较差别无统计学意义(P〉0.05);CH中血管内皮细胞FAD/FAD+NADH(氧化还原比率)分别明显高于CM及NS,差异均有统计学意义(P〈0.01),CM与NS比较差别无统计学意义(P〉0.10)。结论双光子激发荧光(TPEF)结合二次波生(SHG)的非线性光谱成像技术是一种可对婴幼儿毛细血管瘤与毛细血管畸形进行早期无创诊断、鉴别,从而指导治疗的重要临床辅助技术,并具有巨大的应用前景。 Objective To investigate the potential value and feasibility of nonlinear spectral imaging method for real-time and non-destructive detection and diagnosis of capillary hemangioma and capillary malformations among infants in clinic. Methods A total of 36 cases of surgical specimens of the skin capillary hemangioma (CH) and capillary malformations (CM) among infants from September 2012 to November 2013 in our hospital were selected, including 22 cases (2-12 months) of CH and 14 cases (4-12 months) of CM. All of the tissue section samples had 30 μm thickness, and normal skin was as the control group. All samples were analyzed by nonlinear spectral imaging system. Results In the same imaging depth and optimal excitation wavelength, there was a difference between normal and diseased skin tissue in terms of skin collagen content; the elastic fiber diameter in CH was significantly lower than that in the CM and NS, the difference was statistically significant (P〈0.05, P〈 0.01), and the differences between CM and NS was not statistically significant (P〉0.05); the ratio of FAD/FAD+NADH (the redox ratio) of vascular endothelial in CH was significantly higher than that in CM and NS, the difference Was statistically significant (P〈0. 01), and the differences between CM and NS was not statistically significant (P〉0. 10). Conclusion Nonlinear spectral imaging based on TPEF/SHG may be used for early and non-destructive differential diagnosis in capillary hemangioma and capillary malformations among infants, as well as a powerful tool for guide treatment, and has great application prospects.
