导丝“抓捕”技术在下肢深静脉血栓形成介入治疗中的应用 被引量:11

摘要 目的探讨导丝"抓捕"技术在下肢深静脉血栓介入治疗中的应用价值,为临床治疗提供依据。资料与方法选取36例左下肢深静脉血栓患者,穿刺患侧股静脉成功后,于4F单弯导管配合下将鹅颈抓捕器突破左髂总静脉闭塞段至下腔静脉内抓捕经健侧入路的导丝,将其拖至左侧股静脉内,建立右-左股静脉通道,然后沿此通道辅以其他介入手段及术后抗凝溶栓治疗,观察治疗效果。结果36例患者使用导丝"抓捕"技术均获得成功。其中疗效优30例(83.3%),良4例(11.1%),中2例(5.6%),总有效率达100%,治疗优良率为94.4%。所有患者经治疗后血栓均基本溶解,下肢肿胀消退,未发生与插管有关的严重并发症。结论导丝"抓捕"技术可以显著提高左髂总静脉闭塞患者的介入治疗成功率,具有较好的可操作性。 Purpose To explore the application of interventional guidewire "capture" technique in interventional therapy of lower extremity deep venous thrombosis (LEDVT).Materials and Methods Thirty-six patients with left LEDVT were included in the study.After successful puncture of left femoral vein,a gooseneck snare passed the occluded segment of left common iliac vein into the inferior vena cava under the support of 4F single lumen catheter to "capture" the guidewire from right femoral vein which was pulled into left femoral vein to establish fight femoral-left femoral passage.Other interventional treatment and thrombolysis can also be performed.Treatment response was observed.Results Technical success was achieved in all 36 patients,with optimal results in 30 cases (83.3%),fair in 4 cases (11.1%),medium in 2 cases (5.6%).The total effective rate was 100%,and 94.4% obtained excellent and good efficacy.After treatment,DVT resolved in all patients with resolution of lower extremity swelling.There was no procedure related complication.Conclusion Guidewire "capture" technique significantly improves treatment success rate in left common iliac vein thrombosis.
