多层螺旋CT血管成像在乳腺癌手术计划中的临床应用研究 被引量:3

摘要 目的:利用128层螺旋CT血管成像对乳腺癌患者受侵腋静脉进行评估,提出新的评判标准,分析该标准的敏感性、特异性及准确率。方法:搜集经病理证实的乳腺癌51例,均行MSCT平扫及两期增强扫描及血管重建成像。术前根据拟定的腋静脉受侵CT评判标准,共有18例入组,经与手术结果对照,分析CT评判标准的敏感性、特异性、准确率、阳性预测值及阴性预测值。结果:MPR除清楚显示病灶的大小、形态、边缘、密度以及邻近结构受侵犯情况等之外,还可清晰显示腋窝淋巴结与邻近腋静脉等受累情况。VR能够更精细地显示腋窝淋巴结与腋静脉间的相互关系;MIP能较好地显示乳腺血管及肿瘤供血动脉。结论:128层螺旋CT血管成像技术能够较好地显示乳腺病灶、腋窝淋巴结、腋静脉及纵隔及肺内情况,具有全面、直观、准确的特点,是乳腺癌手术计划中一种重要的术前评估手段。 Objective: To evaluate newly introduced criteria for axillary vascular of breast cancer with 128-slice helical CT.To analysis the sensitivity,specificity and accuracy of the new criteria by Comparing with surgical results, and further to evaluate the value of CTA images in surgical navigation and the planning.Method:51 patients with histologically confirmed breast cancer,underwent thin-section MSCT before surgery. 18 of 51 patients were finally enrolled in the study by the criteria of CT. Findings on CT were compared with the results of surgery in each patient. The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were also calculated respectively.Result:The MPR imaging not only displayed clearly the sizes,shapes,margins,density of the lesions and the adjoining invaded tissure,but also displayed the axillary node,vascular etc.The VR imaging showed accurately the relationship between the axillary node and the vascular.The MIP imaging displayed the blood vessels of the nomal breasts and the tumors clearly.Conclusion:The CTA imaging techniques of 128-slice helical CT can display the 1esions, lymph nodes,blood vessels of the breasts,the lung and the mediastinum clearly,which seem to have potential clinical application for breast cancer surgery planning.
