摘要
目的:探讨组织多普勒Z指数联合Tei指数对左向右分流型先天性心脏病(CHD)合并肺动脉高压(PAH)患儿右心功能的评定价值。方法:将74例左向右分流型CHD患儿根据有无合并PAH分为两组:CHD未合并PAH组(简称CHD组)及CHD合并PAH组(简称PAH组),对照组为35例健康儿童;应用传统二维多普勒超声心动图测量右室射血分数(RVEF)、三尖瓣E/A值及组织多普勒成像(TDI)技术测定右心TDI-Z指数及TDI-Tei指数。结果:两组CHD患儿右心TDI-Z指数均较对照组减低(P<0.01),PAH组与CHD组右心TDI-Z指数比较差异有统计学意义(P<0.01);两组CHD患儿右心TDI-Tei指数均较对照组升高(P<0.01),PAH组与CHD组右心TDI-Tei指数比较差异有统计学意义(P<0.01)。右心TDI-Z指数与TDI-Tei指数间呈良好负相关(r=-0.596,P<0.01)。结论:右心TDI-Z指数及TDI-Tei指数联合应用可无创、快速、定量评价左向右分流型CHD合并PAH患儿的右心功能状态。
Objective: To assess right ventricular function in patients with left to right shunt congenital heart disease ( CHD ) associated with pulmonary arterial hypertension ( PAH ) by Z index and Tei index of tissue doppler imaging ( TDI ) .Method: A total of 74 patients with left to right shunt CHD were divided into two groups: CHD without PAH group ( CHD group ) and CHD with PAH group ( PAH group ) according to whether associated with PAH, and 35 as heahhy children.The test parameters included right ventricular ejection fraction ( RVEF ) and E/A ratio of tricuspid valve measured by traditional two-dimensional doppler echocardiography, TDI-Z index and TDI-Tei index of right heart measured by TDI.Result: Compared with the control group, TDI-Z index of both CHD groups reduced (P〈0.01) . There was statistical significance in TDI-Z index between CHD group and PAH group ( P〈0.01 ) .Compared with the control group, TDI-Tei index of both CHD groups increased ( P〈0.01 ) . There was statistical significance in TDI-Tei index between CHD group and PAH group ( P〈0.01 ) .The TDI-Z index showed excellent negative correlation to TDI-Tei index (r=-0.596, P〈0.01 ) .Conclusion: The combination index of TDI-Z index and TDI-Tei index of right heart is a non-invasive, rapid, quantitative way on evaluating the right ventricular function in patients with left to right shunt CHD associated with PAH.
