河南省耐药结核分枝杆菌embB 306密码子突变特征研究 被引量:1

Study on the mutation characteristics of embB codon 306 in drug-resistant mycobacterium tuberculosis isolates in Henan

摘要 目的了解河南省耐药结核分枝杆菌临床分离株乙胺丁醇(EMB)耐药相关基因embB 306的突变情况及不同耐药谱菌株的embB 306基因的突变特征。方法选择2007年-2010年河南省临床分离结核分枝杆菌,经比例法药敏试验,筛选出155株耐药菌株,13株敏感菌株,PCR扩增embB 306基因片段,并进行测序分析,对MDR株、XDR株、一线药物全耐株、对EMB耐药的耐药株、EMB敏感耐药株、全敏感株等不同耐药谱的结核分枝杆菌的embB 306基因突变的差异进行分析。结果扩增均获得202 bp片段,经序列测定比较,全耐药株和对EMB耐药的耐多药菌株中均检测到embB 306密码子的点突变,由ATG突变为ACG;对EMB敏感的耐多药菌株中,有2株检测到embB 306的突变;而在全敏感株中,未检测到基因突变。结论结核分枝杆菌乙胺丁醇耐药相关基因embB 306突变与耐多药(MDR)特别是广泛耐药(XDR)有相关性,但embB 306突变不一定导致EMB耐药。 Objective To observe the mutations of embB gene in ethambutol-resistant mycobacterium tuberculosis(MTB)strains from Henan and the characteristics of embB 306 gene mutant strains in different drug-resistant spectrum.Methods The mycobacterium tuberculosis isolates were selected in Henan province from 2007 to 2010. The drug susceptibility of these clinical isolates was determined by proportion method. There were 155 drug-resistant strains and 13 sensitive strains,and embB 306 gene fragment of the 168 isolates were amplified by PCR and sequenced. The different drug-resistant mycobacterium tuberculosis strains including multi-drug resistant(MDR) isolates,extensive-drug resistant(XDR) isolates,those resistant to all first-line drugs,EMB-resistant strains,EMB-sensitive resistant strains and all drug-sensitive strains were collected to analyze the embB 306 mutation difference. Results 202-bp amplification fragments were obtained from all clinical isolates. embB codon 306 point mutations were detected in mycobacterium tuberculosis isolates resistant to all first-line drugs and EMB-resistant MDR strains by sequencing from ATG to ACG mutation. Two EMB-sensitive MDR strains were detected embB 306 mutation. Gene mutation was not detected in the 13 drug-sensitive strains. Conclusion EMB-resistant related gene embB 306 mutation in mycobacterium tuberculosis has correlation with multi-drug resistance,especially extensively drug-resistance(XDR). But embB 306 mutation does not necessarily lead to EMB resistance.
