摘要
目的探讨实时超声弹性成像的弥散定量分析功能对梗阻性黄疸患者的定量化诊断价值。方法选择梗阻性黄疸患者86例(黄疸组),根据临床总胆红素水平分为轻度黄疸亚组(43例)、中度黄疸亚组(34例)和重度黄疸亚组(9例);另选取同期收治的无黄疸或隐形黄疸患者35例(无黄疸组)。应用超声诊断仪所带的组织弥散定量分析软件对实时超声弹性图像进行分析,测定并记录所有患者肝脏感兴趣区域(ROI)内的应变均值(MEAN)和蓝色领域百分比(%AREA)。比较各组间的MEAN和%AREA,分析MEAN、%AREA与黄疸程度的相关性。结果黄疸组的MEAN和%AREA分别为110.04±6.77和20.72±7.90,与无黄疸组的115.99±5.42和14.25±5.84比较,差异均有统计学意义(P值均<0.05)。轻度、中度、重度黄疸亚组的MEAN分别为112.28±4.60、108.69±7.21、104.49±9.56,轻度、中度、重度黄疸亚组的%AREA分别为18.83±8.16、21.81±6.74、25.60±8.77。无黄疸组和轻度、中度、重度黄疸亚组4组间两两比较,除中度黄疸亚组与重度黄疸亚组间MEAN和%AREA的差异无统计学意义(P>0.05)外,其余各组间的差异均有统计学意义(P值均<0.05)。Spearman等级相关分析显示,MEAN值与黄疸程度呈负相关(r=-0.5,P=0.000),%AREA与黄疸程度呈正相关(r=0.5,P=0.000)。结论实时超声弹性成像的弥散定量分析功能可反映梗阻性黄疸的程度,MEAN和%AREA可作为评估梗阻性黄疸程度的定量化参考指标,其与传统超声检查方法相结合,有望成为全面反映梗阻部位和黄疸程度的超声检查新方法。
Objective To investigate the quantitative diagnostic value of real-time ultrasound elastography with strain Histogram measurement function for obstructive jaundice. Methods Eighty-six patients with obstructive jaundice (jaundice group) were enrolled in the study and divided into mild (n = 43), moderate (n = 34), and severe (n = 9) subgroups according to total bilirubin level. Moreover, 35 patients without jaundice or with invisible jaundice were recruited into non-jaundice group. All patients underwent real-time ultrasound elastography, and the average relative strain value (MEAN) and blue area percentage (% AREA) in the region of interest (ROD of liver were measured by Strain Histogram Measurement Function. The liver MEAN and % AREA were compared between groups. The correlation between MEAN, % AREA and jaundice degree was analyzed. Results The liver MEAN and % AREA were 110. 04±6. 77 and 20.72±7. 90 in the jaundice group, and 115.99±5.42 and 14.25±5.84 in the non-jaundice group; there were significant differences in the MEAN and % AREA between the two groups (both P〈0.05). The MEAN was 112.28 ± 4.60, 108.69 ±7.21 and 104.49 ±9.56 in the mild, moderate and severe subgroups, respectively. The %AREA was 18.83+8.16, 21.81 ±6.74 and 25.60± 8.77 in the mild, moderate and severe subgroups, respectively. There were significant differences in the MEAN and % AREA between non-jaundice group and jaundice subgroups (all P〈O. 05), not between the moderate and severe subgroups (both P〉0.05). Spearman' s correlation analysis showed a negative correlation between the MEAN and the jaundice levels (r = - 0.5, P = 0. 000) and a positive correlation between the % AREA and the jaundice levels ( r = 0. 5, P = 0. 000). Conclusion Real-time ultrasound elastography with Strain Histogram Measurement Function can reflect the degree of obstructive jaundice.
