摘要
目的应用组织多普勒成像(TDI)测量Tei指数(TDI-Tei指数),评价其对不同血糖水平患者左心室功能的临床价值。方法 103例患者根据血糖结果分为四组:A组(血糖正常组,n=25)、B组(新增空腹血糖受损组,n=22)、C组(糖耐量减低组,n=25)、D组(糖尿病组,n=31)。常规二维超声心动图检测,获得左心房内径(LA)、左心室射血分数(EF)、左心室短轴缩短率(FS),二尖瓣血流舒张早期峰值流速(E)及其与舒张晚期峰值流速(A)的比值(E/A)。利用TDI技术测量心尖四腔心二尖瓣环后间隔、侧壁舒张早期运动速度(Em)、舒张晚期运动速度(Am),计算Em/Am、E/Em及Tei指数。比较四组各指标的差异,探讨各参数对不同血糖水平患者左心功能评估的价值。结果糖尿病组TDI-Tei指数较其他三组增大,四组比较差异有统计学意义(P<0.05)。结论 TDI-Tei指数可以简便、快捷、有效、全面地评价不同血糖水平患者的左心室整体功能,是临床早期发现高血糖患者亚临床心功能异常有价值的指标。
Objective To measure Tel-index with tissue doppler imaging(TDI)and assess its clinical value on left ventricular function in patients with different plasma glucose levels. Methods A total of 103 patients presen- ted to outpatient department or admitted at the first time between March 2011 and March 2013 were enrolled in this study. The patients were divided into four groups according to fasting plasma glucose levels:normal plasma glucose group( group A, n = 25 ), newly diagnosed impaired fasting glycemia group( group B, n = 22), impaired glucose tol- erance group(group C, n =25), diabetic group(group D, n =31 ). Left atrial diameter( LA), left ventricular ejec- tion fraction(EF) , left ventricular fractional shortening(FS) , peak velocity of early diastolic mitral flow(E) and the ratio of E to peak velocity of late diastolic mitral flow ( A ) ( E/A ) were measured by conventional two-dimensional echocardiography. Under the apical four-chamber view, the early diastolic motion velocity (Em) and late diastolic motion velocity(Am) of post septum and side wall of mitral annulus were detected by TDI technique, as well as the Em/Am, E/Em and Tel-index were calculated. The difference of each parameter in four groups was respectively compared, and the value for assessing left ventricular function in patients with different fasting glucose levels was ex- plored. Results The TDI-Tei index in group D increased compared with the other three groups, and the difference of TDI-Tei indexs in the four groups had statistical significance ( P 〈 0.05 ). Conclusions TDI-Tei index has sim- ple, fast, effective and comprehensive advantages for evaluating left ventricular global functions in patients with dif- ferent plasma glucose levels, and thus it is a valuable index for early discovering sub-chnical dysfunction of cardiac function in patients with hyperglycemia.
